We
are looking for an organised and patient-focused Care Coordinator (Prescribing
Support) to join our Primary Care Network (PCN) team as part of the Additional
Roles Reimbursement Scheme (ARRS). This is a non-clinical role focused on
improving medication-related support for patients with complex needs, helping
to streamline prescription processes and enhance personalised care planning.
You
will work closely with clinical staff, including pharmacists and GPs, and
support patients in navigating prescription services, especially those managing
long-term conditions or taking multiple medications. This is not a pharmacy
technician post but one that complements clinical roles through effective
coordination and communication.

This vital role bridges patient-facing services and prescription administration, directly supporting GPs and Clinical Pharmacists to streamline medication processes and improve patient care.

This role may require working across all five practices within the Primary Care Network.
Key Responsibilities
Patient-Facing and Coordination Duties:
- Provide tailored, non-clinical support for patients on multiple medications or with complex prescribing needs.
- Help patients understand and manage their prescriptions and signpost to appropriate services.
- Identify individuals requiring Structured Medication Reviews (SMRs) and help book and follow up appointments.
- Support the navigation of prescription systems including NHS App, repeat requests, and digital tools.
- Facilitate shared decision-making and maintain accurate personalised care records.
Administrative and Communication Duties:
- Handle repeat prescription requests and assist in the smooth running of prescription workflows.
- Monitor and track medication-related queries and actions within the clinical system.
- Participate in MDT (multidisciplinary team) meetings, preparing relevant patient information and recording follow-up actions.
- Liaise with pharmacies, care homes, and other community providers to ensure joined-up care.
- This is a role that directly supports GPs and Clinical Pharmacists by relieving administrative burden

Essential
- Have experience in general practice, care coordination, pharmacy, or social care.
- Demonstrate strong communication and digital skills.
- Understand NHS primary care workflows and patient pathways.
- Be confident working across organisational boundaries and engaging with patients and carers.
Desirable
- NVQ Level 3 in Health, Social Care, or Business Administration
- Prior involvement in medication-related patient support, especially in care homes or with vulnerable populations.
- Experience using EMIS clinical system
