Job summary
An exciting opportunity has arisen for
an enthusiastic Nurse Practitioner with an interest in elderly care to join our
friendly practice team working primarily supporting our patients living within
the care home setting with the support of a Lead GP.
We firmly believe the best patient care
needs to be delivered with continuity in mind. This is therefore a new role focusing
on the care and wellbeing of the residents in our two care homes which are
primarily for dementia patients. We are
building an MDT approach with support from a Lead GP, medicines support from
our Clinical Pharmacist and Pharmacy Technician and admin support from our dedicated
administration team. This role would
suit an experienced Nurse Practitioner with a Community Matron background.
This role will be based working in our
two main care homes two mornings a week (8 hours) with protected time to
discuss cases and clinical support from a Lead GP whilst you establish yourself
within the role. There may be a further
opportunity to develop your clinical skills (aside from this role) working in
the practice alongside the Duty Doctor managing acute patients presenting on
the day.

Main duties of the job
Complete ward rounds of the Practice's two main care homes
Deal with acute on the day presentations
Long term condition management
Liaising with Lead GP and MDT team, including pharmacy team and administrators to provide the best care for the patient
Build good working relationships with the care home team
