Job summary
We are seeking an experienced and proactive Practice Nurse to join our well-established GP practice, with a strong focus on chronic disease management and QOF delivery.
Main duties of the job
You will play a key role in delivering high-quality, evidence-based care to patients with long-term conditions, supporting the practice in achieving and maintaining Quality and Outcomes Framework (QOF) targets while improving patient outcomes.
About us
Our practice is situated on the outskirts of Crewe with 11,000 patients.
We have a team of staff dedicated to patient care including 5 partners - 4 clinical and 1 non clinical. 5 Salaried GPs, 1 ACP, 1 Trainee ACP/Clinical Nurse Manager, 4 Practice Nurses, 4 GPAs and an extensive administration team who support the running of the practice.
We also have PCN ARRS staff including Pharmacists, MSK FCPs and a Mental Health Practitioner.

Key Responsibilities
- Lead and deliver chronic disease clinics in line with QOF requirements, including:
- Diabetes
- Asthma and COPD
- Hypertension and cardiovascular disease
- Undertake structured annual reviews and care planning for patients with long-term conditions
- Support achievement of QOF indicators through accurate coding, recall systems, and patient engagement
- Use EMIS Web to maintain precise, up-to-date clinical records and ensure appropriate coding
- Work collaboratively within the multidisciplinary team, including GPs, GPAs, pharmacists, and PCN colleagues
- Participate in audit, clinical governance, and service improvement initiatives
Core Practice Nursing Duties
- Cervical cytology (smears) in line with national screening programmes
- Childhood and adult immunisations, including travel vaccines
- Contraception
- HRT
About You
- Registered Nurse with current Nursing and Midwifery Council (NMC) registration
- Demonstrable experience in primary care and chronic disease management
- Strong understanding of QOF indicators and long-term condition management pathways
- Competent in using EMIS Web for documentation, coding, and recalls
- Skilled in patient education and promoting self-care
- Desirable: Training in diabetes care, respiratory disease, or cardiovascular management
