Job summary
Rural Rother Primary Care Network is looking for an additional Pharmacy technician to join our friendly PCN team. We are a proactive group of rural practices, all practices have a good ratios of GPs to patients. This role is about providing additional proactive clinical support to patients. Currently we are expanding our team to include a third pharmacist and another pharmacy technicians. Most of our practices are dispensing practices, you will be required to work alongside our existing dispensary teams. However your role will be about clinical management of patients and safety, you will not be required to be involved in the daily dispensing activities. We have two GP trainers in the PCN who are provide regular mentoring and supervision sessions for our Allied professionals. We promote ongoing training to support you with developing your relevant skill sets.
Main duties of the job
This a hybrid role, working partly in practices, in our PCN office, in community outreach centres and at home. You will be required to support practices with a variety of activities including, supporting management of shared care agreements, reviewing discharge letters and ensuring medications are updated. Working on our hypertension project assessing and undertaking blood pressure monitoring clinics. You will also be working with the PCN team to develop proactive care programs to support patients, including elements of personalised care planning for vulnerable patients' in our local communities, monitoring, auditing and advising on safe working practises and implementing improvement programs..

Overview of your organisation
Our team currently consists of 2 Care coordinators, 2 social prescribers, an 2 ANP, 2 physios and 3 pharmacy Techs. We are a supportive and friendly team. All teams members are employed by the PCN and deployed to practices to work on designated and agreed projects. Our practices all work well together and regularly meet at our weekly Practice manager forum and our monthly GP lead Quality and innovation group meetings to review the development and success of PCN resources and deployment of schemes
