Job summary

The post-holder will be responsible for providing support to GP practices and care homes to optimise medication use and improve the quality and cost-effectiveness of prescribing and medicines.

Main duties of the job

The successful candidater will work as an integral part of the Shropshire Rural Alliance PCN to support the PCN member practices to deliver the pharmaceuitcal aspects of the Primary Care Network Direct Enhanced Service (DES).

Job description

Job responsibilities

The key requirements of the role include:

  • To support the delivery of an effective, efficient, and safe medicines management service within the PCN by working closely with the wider multidisciplinary team
  • To ensure service compliance with medicines legislation, in particular controlled drugs legislation
  • To provide input into policy, procedures, audit, and research affecting the use of pharmaceuticals at GP surgeries where appropriate
  • To undertake accurate medicines reconciliation to assist pharmacists in structured medication reviews
  • Helping patients to get the best from their medicines. This will include working towards approved protocols, improving repeat prescribing processes, promotion of repeat dispensing, minimising clinical risk and aiming to reduce wasted medicines
  • Provide pharmaceutical services to designated Care Homes, supported and supervised by a pharmacist, to ensure residents are managed safely and medically optimised
  • Support the PCN in achieveing local and national requirements e.g., DES contract, IIF, NICE and QOF
  • To work with GPs, Nurses, and Pharmacists to support and inform the management of prescrtibing practice
  • To undertake thorough reasoned analysis on possible medication switches, which maintain the quality of therapy of the original drug with a lower average unity cost
  • To undertake anlyses of prescribing processes which maintain or increase patient safety but also elimiante waste and duplication
  • To assess and facilitate patient concordance with prescribed medicines
  • Deal with medication related queries from patient and staff on a day-to-day basis
  • support improvements in clinical care through practice-based audit and implemnting change
  • To record all relevant pharmaceuitcal information in the patient's medical notes and interventions in line with departmental policy
  • Lead on medication safety alerts ensuring they are acted on in a timely manner
  • Inform patients regarding changes to their medicines in a professional manner and have the capacity to support and reassure patients when necessary, dealing with any issues or concerns that may arise within the boundaries of the post
