Major Duties and Responsibilities
of the Post
Coding & Workflow
Understand
and act upon clinical information including Pathology results received by
having an understanding of medical terminology and results thresholds clearly
laid out in Standard Operating Procedures and Protocols
Be
responsible for the identification and selection of appropriate Clinical Codes
for any items flagged for coding either as ongoing work or as requests via
tasks from clinicians.
Be
responsible for reading and analysing clinical correspondence received and
determining the necessary actions,
ensuring that the documents are completed and filed on the patient record and
any necessary actions are put in place
Process
clinical correspondence safely. Pass onto GPs and/or other members of the
practice team letters that require further input or action such as
prescriptions or referrals.
Identifying
potential system and process improvements within the practice and the wider
healthcare system that support patients getting the best outcomes in terms of
their care.
Identify
complex or high risk areas that require input or further knowledge from the
clinical team.
Ensure
that any shortfalls in capacity or increases in demand are escalated so that
correspondence is processed within the agreed timescales
Medical Record Summarising:
Incoming paper records
Read
and process all hospital letters (discharge letters/outpatient reports, results
etc) and transpose by coding any information onto the patients clinical record
Code
and link each problem appropriately as per the practice summarising
protocol. Review and enrich the summary page.
Review
QOF (Quality and Outcome Framework) performance markers and ensure read coding
captures these codes
Identify
gaps or errors in medical records and rectify as appropriate, or flag these
gaps, if unable to find any data
Record
all allergies and sensitivities
Alert
recall clerk to any potential recalls that need setting up/reviewing
Ensure
that information on safeguarding children (or vulnerable adults) is
appropriately flagged and identified
Incoming electronic records - GP2GP
Match
electronic record on computer
Validate
all existing medical history (immunisations, problems, etc) entered by previous
practice and complete any gaps
Tidy
home page removing old and out of date reminders, current and past medical
history
Check
medication, allergies and sensitivities in degraded GP2GP data.
Record checking and
verification
Check
medical record content for online coded record access (where patient has
applied)
Report
findings back (via task) to usual GP
Check
medical records (problems and summary items) for patients choosing to have
enhanced summary care record. Make any required amendments for accuracy. Read
Code that checking has been performed as per the protocol.
General
Be
able to work unsupervised whilst also being an effective team member,
communicating and supporting colleagues in achieving the practice aims
Liaise
and communicate with wider practice summarising team and participate in
meetings as required
Attend
relevant training to meet the requirements of the role and improve skills
Maintain
data security at all times (clear desk procedures, lock out computers etc)
Be
aware of information governance standards and policies
