Job summary
We are looking for
an experienced, enthusiastic, friendly and professional Practice Nurse to join our team at
Pontesbury & Worthen Medical Practice on a permanent contract basis, for around 17 hours per week over 2 days. Applicants must have experience of working a GP Practice, and must have completed their fundamentals training.
The successful
applicant would be working alongside our current nursing team, and be responsible for the delivery
of practice nursing services to our patient population. The role will include
managing areas such as health promotion, chronic disease management, health
prevention, as well as supporting the nursing, clinical, and management teams
in achieving QOF targets.

someone with good primary care experience, and an enthusiastic team
player who will, as well as being patient focused, will work well with the
whole practice team and maintain a friendly but professional conduct at all
times. We are a hard working and friendly team who pride ourselves on
delivering excellent patient care.

Pontesbury & Worthen Medical Practice is a developing and growing practice with two sites, situated in the rural villages of Pontesbury and Worthen, just south of Shrewsbury providing medical care for approximately 10500 patients. - Identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects on patients health
- Implement and evaluate individual treatment plans, manage chronic disease registers and run clinics for chronic disease patients
