Job summary
This role involves working within our ‘Same Day Care’ department which is responsible
for triage, urgent care, minor illnesses and injuries.
The post holder will
host clinics supported by a qualified nurse to triage, assess and treat
patients who present with urgent ailments.
They will demonstrate safe and
effective clinical decision making in evaluating which patients need urgent
treatment and providing a fast and thorough investigative response and
treatment plan.
Clinics will be held alongside triage to provide routine
appointments for minor illness, injury and mental health issues. This role will be fully supported by an in
practice GP at all times.

We
are looking for a dynamic individual who will work within our ‘Same Day Care’
department which is responsible for triage, urgent care, minor injuries and
illness.
An ANP, Minor Illness and Prescribing qualification is essential, and
experience of mental health, minor injuries and children is beneficial.

forward-thinking practice. Scott Road is a busy town centre GP practice with a
list size of just under 11,000 patients. Our vision is simple; To achieve the
best possible health and wellbeing for our patients; in order to do this we are
looking at redefining our staffing model to create a position for a talented
ANP.
We
have an incredibly supportive team of 4 Partners and 3 Salaried GPs and provide
a full range of services under the PMS Contract which include minor surgery,
sexual health, maternity services, and host a wide range of on-site activities
such as retinal screening, social prescribing, first contact physio, counselling,
respiratory clinics, diabetes prevention programme and long-term condition
mental health worker.

Job responsibilities
- Assess, diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting with an undifferentiated diagnosis.
- Prioritise patient lists based on presenting history and level or urgency. Where needed liaise with emergency services and hospital wards to facilitate transfer of patient to secondary care.
- Provide physical and psychological clinical examination and devise care and treatment plan in line with current best practice
- Identify patients with complex needs and those at risk of developing long term conditions. Where appropriate use interventions and known methods/treatments to reduce risk and promote health.
- Prescribe medications for treatment of acute illness and evaluate effectiveness of treatment plan for chronic illness.
- Work in line with current recommendations and the latest evidence based medicine to provide optimal treatment to patients.
- Complete administration tasks including interpreting results in a quick and efficient manner to ensure patients have access to further diagnostic tests, referrals and treatment.
- Give telephone advice to allow patients to self-help and sign post to third party organisations.
- Run appointment clinics for long term conditions and chronic disease management.
- Attend routine and urgent home visits within the scope of capability; to include visiting patients at care homes or specialist centres.
Quality Improvement
- Work with the management team to deliver set quality improvement projects.
- Undertake analysis of key clinical areas and make robust plans for delivering improvement.
- Use current thinking and evidence based medicine to ensure all protocols are up to date.
- Comply with QOF regulations and aim to ensure your service delivers on the specific requirements.
- Support and participate in shared learning across the practice and wider organisation.
- Keep up to date with local and national incentives and ensure this is effectively communicated and actioned within practice.
- Review own practice regularly and engage with mentoring to ensure continuous clinical and personal development
