Job responsibilities
OVERVIEW
The post holder will, acting within their professional boundaries,
provide care for the presenting patient from initial history taking, clinical
assessment, diagnosis, treatment and evaluation of care. They will demonstrate
safe, clinical decision making and expert care, including assessment and
diagnostic skills, for patients within the Practice. They will work as an
independent prescriber, providing advice and support for both patients and
colleagues. The post holder will demonstrate critical thinking in the clinical
decision making process. They will work collaboratively with the general
practice team to meet the needs of patients, supporting the delivery of policy
and procedures.
The successful applicant will help integrate patient care across
traditional NHS interfaces, provide clinical leadership as part of the Practice
team and will possess excellent communication and organisational skills.
MAIN PURPOSES
Responsible for
attending to patients who wish to be seen by a health care professional;
diagnosing and treating patients presenting with minor illness; seeing and
advising people in respect of their continuing medical needs. Review, triage
and act on the needs of patients when assessing them via telephone or online
consultation. Make any necessary referrals for investigations in the
appropriate manner. Patients with problems needing referral to secondary care
should be discussed with the registered GP before making such referral.
The role will
also entail a degree of the undertaking of and participation in chronic disease
management and audit, data capture and providing patient summaries and reports.
The role will involve reviewing, triaging, assessing and acting on online
consultation submissions.
CORE RESPONSIBILITES
The following are the core
responsibilities of an Advanced Clinical Practitioner. There may be on
occasion, a requirement to carry out other tasks; this will be dependent upon
factors such as workload and staffing levels:
- Practice
as an independent prescriber working within scope of practice
- Carry
out face-to-face and telephone consultations with patients with minor
illnesses
- To
provide holistic, high quality, autonomous clinical care working at all
times in accordance with professional guidelines and scope of competence.
- To be
involved in the management of patients with chronic disease and minor
illness assessing and evaluating their care where indicated using
appropriate recall systems to actively signpost the patient to the correct
professional
- To
treat patients with acute illness independently in core clinics.
- To make an assessment of the
patients physical, psychological and social needs; examine, diagnose and
arrange treatment, including appropriate
prescribing,
discharge, follow up or referral of patients.
- Organise further care if needed eg
rapid response, care packages or transfer to secondary care.
- Agree management plans with the
patient’s GP where required
- Undertake the collection of
pathological specimens including intravenous blood samples, swabs etc.
Perform investigatory procedures requested by the GPs
- Support triage of visit requests
and provide home visits when appropriate
- Record
information fully into the patients’ notes, ensuring contemporaneous,
accurate documentation of all care given to patients.
- Liaise
with other professionals within the Hub to support patients’ care.
- To
provide advice and support for patients with self-limiting conditions
- To
review, triage and respond to online consultation submissions
- Encourage
cost-effective prescribing within the practice
- To
reconcile medicines following patients discharge from hospital or clinic
appointments
- To
re-authorise medication for repeat prescribing within scope of practice
- Identify
patients suitable for repeat prescribing, liaising with the relevant
clinicians as required
- Review
and support safe prescribing systems for vulnerable patients, improving
compliance
- Partake
in vaccination programmes including seasonal flu campaigns
- Provide
clinical advice and support to other professionals within the practice
- To
use EMIS clinical system for data capture and to produce
reports/statistics.
- To
attend and input to the regular practice meetings, as deemed appropriate.
- To
attend regular training/updates identified in your Personal Development to
maintain skills and develop competence in new areas depending on service
needs. Ensure professional knowledge is kept up-to-date.
- Clinical
supervision in line with professional and CQC requirements.
- To
participate in research within the Practice as required.
- To
assist in formulating practice protocols/guidelines.
- To
play an active role in the continuing development of the practice and
support practice management plans and support QoF and Enhanced Service
targets
- Any other
duties appropriate to the role.
In addition to the primary
responsibilities, the Advanced Clinical Practitioner may be requested to:
- Partake in audit as directed
- To provide subject matter expertise
on a specialist area of practice according to qualifications e.g.
medication monitoring, womens health, palliative care etc
- Participate in the education and
training of students of all disciplines and the introduction of all
members of the practice staff where appropriate
- Provide education on specialist
area for the clinical team
- Review the latest guidance ensuring
the practice conforms to NICE, CQC etc.
- Support the other clinicians with
clinical matters, establishing positive working relationships
- Support
development of clinical governance systems as required
The post holder will be aware of the
various statutory requirements and will assist in implementation as directed by
the Partners and Practice Manager. There is a need to work closely with
reception and clinical staff to ensure the smooth running of the business,
reporting any problems encountered to the practice manager.
