JOB DESCRIPTION
OVERVIEW
The
post holder will be an experienced Advanced Clinical Practitioner or
Prescribing Nurse who, acting within their professional boundaries, will
provide care for the presenting patient from initial history taking, clinical
assessment, diagnosis, treatment and evaluation of care. They will demonstrate safe, clinical decision
making and expert care, including assessment and diagnostic skills, for
patients within the practice. They will
work as an independent prescriber, providing advice and support for both
patients and colleagues. The post holder will demonstrate critical thinking in
the clinical decision making process.
They will work collaboratively with the general practice team to meet
the needs of patients, supporting the delivery of policy and procedures.
The
successful applicant will help integrate patient care across traditional NHS
interfaces, provide clinical leadership as part of the Practice team and will
possess excellent communication and organisational skills.
In
addition: the post-holder will utilise
their recognised professional knowledge and skills to deliver care to patients
in General Practice and support long term transformation of the primary and
community workforce in line with the strategic objectives of the practice.
MAIN PURPOSES
In
addition to regular minor illness clinics, the role will involve leading on
providing continuous care for our care and nursing home patients, including
undertaking weekly ward rounds, updating care plans, liaising with residential
home staff and family members and working with GPs and the wider MDT team to
navigate care pathways. The role will also entail a degree of the undertaking
of and participation in chronic disease management and audit, data capture and
providing patient summaries and reports. The role will involve reviewing,
triaging, assessing and acting on online consultation submissions.
The
following are the core responsibilities of an Advanced Clinical Practitioner.
There may be on occasion, a requirement to carry out other tasks; this will be
dependent upon factors such as workload and staffing levels:
- Practice
as an independent prescriber working within scope of practice
- Carry
out face-to-face and telephone consultations with patients with minor illnesses
- To provide holistic, high
quality, autonomous clinical care working at all times in accordance with
NMC / GPhC professional guidelines and scope of competence.
- To be involved in the
management of patients with chronic disease and minor illness assessing
and evaluating their care where indicated using appropriate recall systems
to actively signpost the patient to the correct professional
- To
treat patients with acute illness independently in core daily clinics.
- To make an assessment of the
patients physical, psychological and social needs examine, diagnose and
arrange treatment, including appropriate prescribing, discharge, follow up
or referral of patients.
- To
provide medication review services with patients in the practice and during
domiciliary visits to the local nursing home and for housebound patients
- To
provide advice and support for patients with self-limiting conditions
- To
review, triage and respond to online consultation submissions
- Encourage
cost-effective prescribing within the practice
- To
reconcile medicines following patients discharge from hospital or clinic
appointments
- Review
medications for newly registered patients
- Carry
out targeted medication reviews for high-risk patients
- To
re-authorise medication for repeat prescribing within scope of practice
- Identify
patients suitable for repeat prescribing, liaising with the relevant clinicians
as required
- Review
and support safe prescribing systems for vulnerable patients, improving
compliance
- Ensure contemporaneous,
accurate documentation of all care given to patients.
- Provide clinical advice and
support to other professionals within the practice
- To use EMIS
clinical system for data capture and to produce reports/statistics. To attend
and input to the regular practice meetings, as deemed appropriate.
- To attend
regular training/updates identified in your Personal Development To maintain
skills and develop competence in new areas depending on service needs. Ensure professional knowledge is kept
up-to-date.
- Clinical
supervision in line with professional and CQC requirements.
- To participate
in research within the Practice as required.
- To assist in
formulating practice protocols/guidelines.
- To play an
active role in the continuing development of the practice and support practice
management plans and support QoF and Enhanced Service targets
- Any other duties
appropriate to the role.
In addition
to the primary responsibilities, the Advanced Clinical Practitioner may be
requested to:
- Partake
in audit as directed
- To
provide subject matter expertise on a specialist area of practice according to
qualifications e.g. medication monitoring, womens health, palliative care etc
- Provide
education on specialist area for the clinical team
- Review
the latest guidance ensuring the practice conforms to NICE, CQC etc.
- Support
the other clinicians with clinical matters, establishing positive working
relationships
- Support
development of clinical governance systems as required
The
post holder will be aware of the various statutory requirements and will assist
in implementation as directed by the Partners and Practice Manager. There is a
need to work closely with reception and clinical staff to ensure the smooth
running of the business, reporting any problems encountered to the practice
manager.
Organisational
Relationships:
A
sound professional working relationship will be required with the clinical and
non-clinical staff working within the Practice as well as those in community
services, secondary care, care homes, etc.
In General:
Advanced
Clinical Practitioners must be on the relevant professional register e.g. NMC
or GPhC Previous experience working at Advanced Clinical Practice is
necessary.
A
pleasant disposition and approachable manner are key attributes, together with
sound communicative and interpersonal skills.
The post requires the ability to exercise tact and discretion, and to
use initiative. The ability to work
under pressure and as part of a team are equally important.
The
successful applicant will be able to prioritise their work and meet
deadlines. They will be
computer-literate and possess the necessary IT skills to work confidently with
electronic information.
It
will be necessary to hold a full driving licence and to have own transport to
travel the short distance necessary when undertaking visits to care homes and
housebound patients.
The
Practice will support the continuing education and professional development of
Advanced Clinical Practitioners in the form of attendance at study days,
courses and with in-house training, as deemed appropriate for the benefit of
both the individual and the Practice.
Training
requirements will be monitored by yearly appraisal. Personal development will
be encouraged and supported by the Practice. It is the individuals
responsibility to remain up to date with recent developments, especially all
aspect of the GMS contract and national requirements and guidelines.
