Job summary
We are looking for a Clinical Pharmacist with Independent Prescribing qualifications and experience to
join our team, who will work alongside our in house PCN pharmacist in
supporting the GPs in the provision of prescribing prescriptions, chronic disease and meds management.
The post holder will
take responsibility for areas of medicines optimisation as a prescriber within the practice and
undertake structured clinical medication reviews to proactively manage patients
with complex polypharmacy. The post holder will provide clinical leadership on
medicines optimisation and quality improvement (QI) and manage some aspects of
the quality and outcomes framework and enhanced services at practice.
Demonstrating
improvement in patient outcomes and working to reduce health inequalities within
the Practice, contributing to improving the efficiency of general practice and
delivering a best in class service. The role will involve playing a full
and active part of the practice clinical multidisciplinary team.
