Cheylesmore Surgery is expanding it's clinical team and is looking for an experienced and motivated Clinical Pharmacist to join its existing multidisciplinary team.
We are happy to consider 1 Full Time Clinical Pharmacist or 2 Part Time Clinical Pharmacists.
£28-£35 per hour dependant on experience.
We are looking for a Clinical Pharmacist who is an independent prescriber and experienced in dealing with patients with minor illness (see & treats).
Main duties of the job
The Clinical Pharmacist will work alongside our established clinical team to support the assessment and management of minor/common/self-limiting illness (consulting with patients with differentiated and undifferentiated illness), chronic disease and medicines optimisation.
Specifically, the clinical pharmacist will work autonomously within an extended role (within own competency) to provide a combination of on the day appointments, minor illness (see & treats) alongside chronic disease management/clinical medication reviews to proactively manage patients and provide pharmaceutical advice to the practice population. They will support the repeat prescription service, deal with acute prescription requests and medicines reconciliation on transfer of care and review systems for safer prescribing.
An independent prescribing qualification is essential - please do not apply if you are not a prescriber.
The Clinical Pharmacist will help patients to get optimal use from their medicines by switching medications to agreed and approved protocols, improving of repeat dispensing and online ordering, minimising clinical risk and aiming to reduce wasted medicines.

- Manage patients with common/minor/self-limiting ailments (consulting with patients with differentiated and undifferentiated illness) while working within scope of practice and limits of competence.
- Increase safety and quality of prescribed medicines.
- Be a point of contact for patients regarding medicines queries
They should support the following clinical work:
- Clinical Medication Review
- Chronic Disease Management (for example, managing hypertension/Type 2 Diabetes/Asthma/COPD/Polypharmacy review)
- Medicines reconciliation on transfer between care providers
They should also support the follow practice development work:
- Focusing on QoF Domains, improving disease registers
- Prescribing Safety, Clinical Audit, Local Enhanced Services support
- Lead on prescription management (systems and processes, ensuring monitoring is being done, support clerical staff).
