Job summary
An exciting opportunity has arisen for an additional part time care coordinator to join East Lindsey Primary Care Network as part of the proactive, falls and frailty Occupational Therapy (OT) team in our south practices. The OT team support patients within East Lindsey Primary Care Network to maintain independence with activities of daily living, reduce the risk of falls, reverse or prevent frailty progression and promote wellbeing. Our aim is to be as proactive as possible to reduce the chance of people reaching crisis situations. The Occupational Therapy Care Coordinator will be invaluable in helping us to deliver these aims.
The role is varied and will include supporting the Occupational Therapists to complete functional assessments and follow-up visits in GP surgeries, patients own homes, care homes and via telephone. You will be involved in supporting to deliver intervention plans which may include the demonstration of exercises prescribed by the Occupational Therapist. You will prescribe basic equipment to aid activities of daily living and support with follow-up assessments once the equipment is in situ. You will also support in the coordination and completion of onward referrals for patients to ensure they receive appropriate, well-coordinated care.
Main duties of the job
Assessments will be within the patient's own home, care homes, GP surgeries, or via telephone. As some assessments are community based, the ability to travel to a variety of venues throughout the PCN is essential
To contact patients via telephone to complete initial templates and book appointments in agreement with an Occupational Therapist.
To help identify patients via SystmOne searches that would be suitable for proactive Occupational Therapy input, under the guidance of an Occupational Therapist.
Supporting Occupational Therapists to carry out functional assessments and follow-up visits within the patients own home, within the service remit
Delivery and fitting (where required) of assistive equipment as directed by the Occupational Therapist and safe moving and handling of the same
To maintain a personal duty of care.
To be responsible for the correct and safe use of the physical environment including equipment and to be responsible for the allocation of these resources
Actively participates in, in-service training opportunities
Effective and accurate communication with patients, families and the multi-disciplinary team regarding outcomes of assessments and visits within scope of the job role
Making onward referrals to health and social care providers as required using appropriate IT systems
Induction and training will be provided along with team support, supervision and training opportunities.

We continue to forge relationships to support the future development of our PCN and our workforce.
Through collaboration with the wider health and care system and voluntary sector, we aim to improve the health and wellbeing of our local communities whilst maintaining their individual values and identities.
