Clinical Pharmacist Full or part-time

AfC 7 8a dependent on experience

We are excited to recruit a Clinical Pharmacist to join our PCN and work within our team of 3 practices, mainly covering the Amesbury Area. You will be working within a small pharmacy team as part of our wider multi-disciplinary team.

Full time, 37.5 hours or part-time are offered. Pay can be negotiated in accordance with experience and qualifications.

To be appointed to this role it is essential that you: Must be enrolled in, or have qualified from, an approved 18-month training pathway or equivalent that equips the Clinical Pharmacist be able to practice and prescribe safely and effectively in a primary care setting (for example, the CPPE Clinical Pharmacist training pathways).

Will be working with a multiple-disciplinary team

You must have the right to work in the UK

Have a full and clean UK Driving licence

Can clinically assess and treat patients using your expert knowledge of medicines for specific disease areas

Be a prescriber, or completing training to become a prescribers

Undertake clinical medication reviews (especially for patients with chronic diseases) and to proactively manage complex polypharmacy, especially older people, people in care homes, those with multiple co-morbidities and people with learning disabilities or autism.

Optimise medicine use, including antimicrobial stewardship guidance and quality improvement, whilst contributing to the QOF

Undertake medication reviews, reducing waste and promoting selfcare

Liaise with community and hospital pharmacy to help improve patient outcomes

Support with production of shared care protocols, clinical research with medicines, liaison with specialist pharmacists (including mental health and reduction of inappropriate antipsychotic use in people with learning disabilities), liaison with community pharmacists and anticoagulation.

Our PCN is a collaborative group of GPs. Our patient groups include armed forces families and many people of Nepalese origin. We have ambitious but pragmatic ideas about the delivery of community healthcare, and a history of successfully working together as a locality. We are:-

The Castle Practice, Ludgershall with a large branch in Tidworth

Between us, we boast five GP trainers plus additional educators for medical students.

Located close to the beautiful and ancient cathedral city of Salisbury, the area comprises rural and semi-rural landscapes plus historic military areas, rail links to London and the South West. You would be embedded within one of our three practices to ensure continuity.

Our CQC ratings are Good and we achieve the Quality and Outcomes Framework standards each year. Our teams are multi-disciplinary comprising GPs, practice nurses, health care assistants, a pharmacy technician, social prescriber, f with administrative and IT support. We operate an open culture and multi-disciplinary meetings to ensure constant learning and improvement.
