Job summary
As a Clinical Pharmacist at St Mary's Surgery, you will play an important role in delivering high quality, patient-centered care. This advanced clinical role offers the opportunity to work autonomously and collaboratively within a supportive multidisciplinary team. If you are new to primary care, then we would be willing to work with you to develop your scope of practice.
Main duties of the job
Patient-Facing Clinical Care:
Lead clinics in areas of scope of practice such as lipid management, diabetes, asthma, and cardiovascular health. Conduct in-depth clinical assessments, interpret diagnostic tests, and develop tailored treatment plans for patients, with a focus on long-term condition management and primary prevention.
Chronic Disease Management: Support the proactive management of patients with chronic diseases, including diabetes, COPD, and hypertension. You will care for patients with complex needs, such as polypharmacy, frailty, or multimorbidity, offering education and empowering them to self-manage their conditions effectively.
Medicines Optimisation and Safety: Champion medicines optimisation initiatives, ensuring evidence-based prescribing practices that align with NHS standards. Conduct structured medication reviews for patients in care homes, those on high-risk medications, or those with multi-morbidity, to enhance therapeutic outcomes and improve patient safety.
Collaborative Multidisciplinary Working: Work alongside a skilled and diverse team, including GPs, registrars, nurses, paramedics, and advanced practitioners.
Quality Improvement and Leadership: Lead quality improvement initiatives, such as prescribing audits, medicines safety reviews, and risk reduction projects. Provide mentorship to pharmacy technicians and take a leadership role in driving best practices for prescribing and medication safety.
