Job summary

We are looking for an enthusiastic and motivated individual to support our growing team. The candidate will be a registered nurse and ideally have experience working in a busy GP practice.

Main duties of the job

Day to day practice nursing duties, including but not limited to:

*Smears

* Travel Health and Vaccinations

*Childhood imms

*Family planning and womens health

*support and lead of practice QOF/NIS/LES work

*Ear syringing

*ECGs

*Care plans

*Smoking cessation

*Minor ailments and wound care

About us

We are a forward thinking practice serving 11500 patients in a purpose built building adjacent to the All Saints DLR.

The successful candidate will join a clinical team of 12 staff including GPs, Clinical Pharmacists, HCAs, PAs and other allied health care professionals.

Job description

Job responsibilities

Chronic Disease Management:

To provide nurse-led clinics for chronic disease management including cardiovascular disease, lung disease, diabetes and other long-term conditions

To work in partnership with patients and their carers to assess individual health needs, plan appropriate care and use every appropriate opportunity to promote health and reduce the incidence of disease

Manage programmes of care effectively through working in partnership with others, optimising health outcomes and resource use and providing appropriate support to patients.

Initiate and carry out programmes of health screening

General Practice Nursing:

To provide routine practice nursing services such as ear irrigation, venepunture, suture removal, wound dressings, ECGs and 24 hour ambulatory blood pressure monitoring

To provide domiciliary nursing care

To administer drugs and vaccines via Patient Group Directions (PGDs) and Patient Specific Directions (PSDs) as appropriate.

To provide seasonal vaccination clinics

To carry out sexual health screens, provide contraceptive advice and smear tests

To identify and assess risks to the health of all new patients registering with the Practice, documenting their health status and counselling them appropriately with referral to a General Practitioner as appropriate.

To undertake diagnostic health screening, health surveillance and therapeutic interventions within a broader health promotion/public health strategy.

To develop and audit nursing standards.

To participate in the ordering of vaccines and dressing supplies

To take responsibility for managing the cold chain and daily fridge checks

To take responsibility for checking and maintaining emergency drugs and equipment

  • A-C GCSE English
  • Adult Nursing Degree
