Main duties of the job

The post holder will be expected to undertake all the normal duties and responsibilities associated with a Care Navigator role working within primary care including but not limited to the following:

  • Process confidential telephone and e-requests for appointments appropriately.
  • Answer incoming phone calls; deal with caller queries efficiently and courteously or transfer calls where necessary.
  • Signpost patients to the correct service; acting as a key liaison between patients, the clinical team and external agencies.
  • Input new registrations/ temporary registrations and relevant patient information as required.
  • Accurately input data into patient healthcare records as necessary ensuring discretion and confidentiality.
  • Maintain a clean, tidy and effective working area at all times. Similarly ensuring patient areas are well kept and free of hazards.
  • Support all clinical staff with general tasks as requested.
  • Demonstrate thorough knowledge of practice policies and procedures, adhere to protocol at all times with particular reference to confidentiality of patient records.

Experience working with Microsoft office and EMIS Web clinical system would be beneficial for this role, however not essential as full training will be given.

About us

We are a friendly and supportive practice with a list size approximately 11,000 patients, based 2.5 miles from Huddersfield town centre and within easy reach of the M62. We pride ourselves on providing a high level of patient care. We are also a keen training practice for both medical students and GP trainees, consistently receiving excellent feedback.

Our team consists of 5 GP partners, 3 salaried GPs, 2 nurses, 1 HCA, GPA and an admin team of Care Navigators, Clinical Coders and Secretary. Our GP's have special interests including diabetes, musculoskeletal conditions, minor surgery, postgraduate education, women's health and children's health. We also work with clinical pharmacists, social prescribers and physician associates through our primary care network.

Job Title: Care Navigator

Reports to: Care Navigator Team Leader/Practice Manager

Job Summary:

The post-holder will:

Provide efficient administrative support services to all members of the practice team.

Receive, assist, and direct patients to access the appropriate service or healthcare professional in a courteous, efficient and effective way.

Project a positive and friendly image to patients and other visitors, both in person and via the telephone.

Job Responsibilities:

General

  • Be familiar with the practice clinical system (EMIS) and accurately input data as required.
  • Have working knowledge of the practice telephone system and handle incoming and outgoing telephone calls in a polite and efficient manner.
  • Ensure all messages are passed to the appropriate team member, and that colleagues are made aware of any action required.
  • Be part of the triage team when requested.

Administration

  • Have a thorough, demonstrable knowledge of all practice policies and procedures.
  • Ensure that all administrative work is carried out in a timely and competent manner.
  • Photocopying, shredding, laminating and other administrative duties as required.
  • Pulling/filing medical records.
  • Carry out data input, ensuring that the recording of all relevant information into the patient's electronic medical record is accurate and concise.
  • Create patient correspondence as appropriate from pre-defined templates.

Medical Notes/Records Summarising

  • Cull and sort patient records and letters in preparation for summarising.
  • Repair/renew patient medical record folders as necessary.

Reception

  • Provide an effective and efficient reception service; flexibility to cover all reception positions as necessary.
  • Keep reception and patient areas tidy and free of hazards at all times.
  • Process repeat prescriptions requests in accordance with practice guidelines.
  • Handle completed general patient admin and clarifying dates of personal and contact information.
  • Communicate clearly with incoming shift staff to handover relevant information on outstanding issues.
  • Registration of new patients (requires knowledge of practice area).
  • Process patient change of address on both digital and medical records (requires knowledge of practice area).
  • Process telephone and in person appointment requests for both on the day and future appointments.
  • Deal with home visit requests and produce any documentation required for the GP's.
  • Arrange and record hospital transport requests.
  • Assist the Practice Manager with clerical and administrative duties.

Other Tasks

  • Undertake post office duties.
  • Any other tasks allocated by the GP Partners, Practice Manager, or Care Navigator Team Leader in line with the pay grade.
