Job summary

We are looking to recruit a motivated, forward thinking and passionate Enhanced Nurse Practitioner, working 14-21 hours per week, to join our expanding nursing team (Lead Practice Nurse, Practice Nurse and 2 HCAs) and deliver high quality patient care to the local population.

Based at The Hall Practice in Chalfont St Peter, this role will be employed by The Chalfonts Primary Care Network (CPCN) which includes three GP Practices and supports a local population of approx. 32,000.

Main duties of the job

We are recruiting an Enhanced Practice Nurse to join our team and ensure that our patients continue to receive the best possible care. Our practice and PCN has a strong culture of training and development.

You will be part of a growing and supportive nursing team led by our experienced lead Practice nurse and you will also work closely with our multi-disciplinary team to provide excellent patient-centred care.

The ideal candidate will have a specialist interest in either Diabetes or Respiratory medicine holding a level 7 postgraduate certification in one or more specialist areas of care.

We are looking for someone who is passionate about change and innovation and can help us to continue to improve the quality of care we provide.

Essential

  • Registered with the Nursing & Midwifery Council.
  • Evidence of other post registration at level 7 or above relevant to their area of enhanced practice.
  • Working at an enhanced level of practice as described in the Primary Care and General Practice Nursing Career and Core Capabilities Framework.
  • Evidence of personal development.

Experience

Essential

  • Understanding and experience of research and audit.
  • Understand the importance of evidence-based practice and able to demonstrate promotion of best practice through clinical governance and learning.
  • Ability to work both with a high degree of autonomy and as part of a team.
  • Ability to work effectively with the wider healthcare community.
  • Critical thinking in decision making.
  • An awareness of current development and initiatives in primary care.
  • An awareness of local commissioning objectives that impact on primary care and the services that can be delivered.

Desirable

  • Experience in General Practice.
  • Evidence of teaching and mentoring junior staff.
  • Evidence of undertaking clinical audits.
