We are looking for a dynamic nurse practitioner who can help with acute and planned care for our busy family practice.

Main duties of the job

Main duties will include acute minor illness clinics, palliative care co-ordination, home visits, planned and unplanned care in general practice. Support to GP team with administritive pathology and patient management

Job responsibilities

Main duties of the job

  • Provide expert clinical assessment and treatment to patients, both face-to-face and remotely
  • Manage acute presentations, chronic conditions, and minor illness independently
  • Prescribe medications safely and appropriately as an independent prescriber
  • Request, interpret and act on diagnostic tests and investigations
  • Refer patients to appropriate services or clinicians as needed
  • Contribute to chronic disease management, QOF achievement, and preventative health
  • Support the training and supervision of other clinical team members (if appropriate)
  • Maintain accurate and timely clinical records in line with practice policy

you will work autonomously to assess, diagnose, treat, and manage patients presenting with a wide range of conditions. Youll be supported by a collaborative multidisciplinary team and benefit from a progressive environment that values innovation and continuous learning.
