Job responsibilities

Job Summary

The Practice Nurse is a vital member of the primary care team, delivering high-quality, evidence-based nursing care to patients across a wide range of clinical areas. This includes health promotion, disease prevention, screening, and the management of long-term conditions. The role involves working autonomously and collaboratively as part of a supportive and dynamic clinical team.

• Conduct chronic disease management clinics (e.g. asthma, COPD, diabetes, hypertension)

• Provide routine and travel immunisations, childhood vaccinations, and flu clinics

• Undertake cervical cytology screening (smear tests)

• Perform wound care, dressings, removal of sutures/clips, and compression therapy

• Provide family planning, sexual health advice, and contraception services (if trained)

• Support NHS health checks and lifestyle advice for smoking cessation, diet, and physical activity

• Conduct blood pressure monitoring, and urinalysis

• Administer injections (e.g. B12, Depo-Provera, vaccines)

• Maintain accurate and up-to-date patient records using clinical IT systems (e.g. EMIS/SystmOne)

• Participate in audits, clinical governance, and quality improvement initiatives

• Ensure infection control and cold chain protocols are followed

• Maintain personal CPD in line with NMC requirements and current clinical guidelines

• Contribute to the development of clinical protocols and care pathways

• Supervise or support HCAs where appropriate

• Work collaboratively with GPs, ANPs, HCAs, admin staff, and other healthcare professionals

• Participate in clinical team meetings, MDTs, and practice reviews

• Deliver patient education and group sessions where appropriate

• Actively support and contribute to a friendly, respectful, and inclusive team culture

• Registered Nurse (NMC registration)

• Experience or strong interest in primary care

• Excellent clinical and communication skills

• Ability to work independently and as part of a team

• Knowledge of safeguarding, infection control, and patient confidentiality

• Training in chronic disease management

• Cervical cytology and immunisation updates

• Experience using clinical systems (e.g. EMIS)

• Mentorship or supervision experience

Qualifications

Essential

• Essential Qualifications & Requirements
• Registered Nurse (Adult) NMC Registration
• Current, active registration with the Nursing and Midwifery Council (NMC)
• Evidence of Continuing Professional Development (CPD)
• Up-to-date knowledge of current nursing practice and clinical guidelines
• Basic Life Support (BLS) training
• Good written and verbal communication skills
• IT literacy
• Ability to use clinical systems (e.g., EMIS) and maintain accurate electronic records
• Post-registration qualifications in one or more of the following:
• Asthma / COPD management
• Diabetes care
• Cardiovascular disease / Hypertension
• Cervical cytology (smear taking)
• Childhood and adult immunisations
• Travel health
• NHS Health Check training
• Wound care or tissue viability training
• Training in contraception / sexual health (e.g., subdermal implant or IUD fitting)

Desirable

• Mentorship or teaching qualification for supporting student nurses or HCAs
• Experience in primary care or community nursing

Experience

Essential

• Current or recent experience in a nursing role (primary care, community, or acute setting)
• Ability to work autonomously and as part of a multidisciplinary team
• Experience in delivering person-centred care and managing long-term conditions, such as:
• Diabetes
• Asthma or COPD
• Hypertension or cardiovascular disease
• Experience with basic nursing procedures, such as:
• Wound care and dressings
• Administration of vaccines and injections
• Cervical smear taking
• Effective use of electronic patient record systems (e.g. EMIS)
• Familiarity with safeguarding, infection control, and confidentiality protocols
• Health promotion and patient education

Desirable

• Previous experience in a general practice or primary care setting
• Running chronic disease management clinics independently
• Childhood and travel immunisation delivery
• Experience in cervical cytology (smear tests)
• Sexual and reproductive health (e.g. contraception advice)
• Conducting NHS Health Checks or lifestyle clinics
• Supervising or mentoring students or HCAs
• Involvement in clinical audits or quality improvement work
• Using templates and protocols within GP clinical systems
