We are looking for a Practice Nurse to join our team. This is a part-time (20-26 hours per week) permanent role.

We are seeking a motivated, dedicated, and enthusiastic practice nurse who is passionate about delivering high-quality patient care and thrives in a supportive primary care environment. Main duties of the job

As a Practice Nurse, you will be responsible for working as part of the Practice Nurse team assisting in the care of patients to include chronic disease management, treatment, preventative care, screening and patient education. You will work collaboratively within our team to provide safe, effective, and evidence-based care to our patient population. You will manage a range of treatment room duties and chronic disease management clinics, supporting health promotion and preventative care initiatives.

Job responsibilities

MAIN DUTIES AND RESPONSIBILITIES.

1. To provide assessment, screening services, health promotion education and management.

i) Provide nursing treatments to patients in collaboration with general practitioners or independently to agreed protocols and as prevailing practice contractual obligations indicate.

ii) To provide general and specific health screenings to the practice patients (within agreed protocols) with referral to general practitioners as necessary.

iii) Advise patients on general health care and minor ailments with referral to general practitioners as necessary.

iv) Assist with chronic disease management according to practice protocols and as skillset determines.

v) Assist with minor surgical procedures including vasectomy and coil fitting.

vi) Undertake childhood immunisations in accordance with national schedules.

2. PATHOLOGICAL SPECIMENS AND INVESTIGATORY PROCEDURES.

i) Undertake investigatory procedures requested by the general practitioners to include the taking of intravenous blood samples,swabs,and other pathological specimens as required.

ii) Undertake cervical cytology and teaching breast self examination.

3. ADMINISTRATIVE AND PROFESSIONAL RESPONSIBLITIES.

Participate in the Administrative and professional responsibilities of the practice team.

i) Ensure accurate notes of all consultations and treatments are recorded in the patients notes on the clinical computer system when appropriate.

ii) Ensure accurate completion of all necessary documentation associated with patient health care and registration with the practice.

iii) Ensure collection and maintenance of statistical information required for regular and ad hoc reports.

iv) Attend and participate in practice meetings as required.

v) Assist in the formulation of practice philosophy strategy and policy, where appropriate.

4RESEARCH PROJECTS.

Co-operate and participate as required in any research projects and audits within the practice.

5EDUCATION, TRAINING and PROFESSIONAL DEVELOPMENT

Participate in self education and training as required and assist with the teaching of attached students as appropriate.

Maintain professional registration and ensure continued education by attendance at courses and study days as deemed useful or necessary for professional development.

Participate in individual annual performance review including taking responsibility for maintaining a record of training and development.

6LIAISON.

Maintain effective liason with other agencies and staff concerned in patient care and with all other disciplines within the practice, with appropriate regard to confidentiality. Liaise with Business Manager with reference to hours of work,clinic times and changes thereto, holidays, courses,etc.

7CONFIDENTIALITY.

Adhere at all times to the practices confidentiality policy and professional confidentiality standards.

8OTHER

Adhere to the practices health and safety policy and support the equality, diversity and rights of patients, carers and colleagues, ensuring that privacy and dignity are respected.
