Clinical Pharmacy
in General Practice is part of an exciting programme of transformation to
develop a new model of care across the Primary Care Network. This model is
supported by national policy including the Five Year Forward View and GP
Forward View to better utilise the role of pharmacy professionals within the
wider healthcare team. Overarching goals are to help keep patients safe and
well out of hospital and reduce the demands on general practice regards
medicine use. The post holder will work within their clinical competencies as
part of a multi-disciplinary team to provide expertise in clinical medicines
management, provide face to face structured medication reviews, manage long
term conditions, management of medicines on transfer of care and systems for
safer prescribing, manage repeat prescription authorisations processes, while
addressing both the public health. The post holder will provide leadership on
quality improvement and clinical audit and well as managing some aspects of the
Quality and Outcomes Framework.
This role is
pivotal to improving the quality of care and operational efficiencies so
requires motivation and passion to deliver an excellent service within general
practice. As well, the post holder will develop and encourage a positive
culture of medicines optimisation principles to assist the practice in
achieving local and national medicines-related objectives.
Main duties of the job
To be part of a
multi-skilled primary care team in a patient facing role providing patient
centred medical services based on patient needs with a focus on continuity of
care. Including: Patient facing / telephone Clinical Medication Review.
Be responsible for the care management of
patients with chronic diseases and undertake clinical medication reviews to
proactively manage people with complex polypharmacy, especially the elderly,
people in care homes, those with multiple co-morbidities and people with learning
disabilities or autism
Review the
on-going need for each medication, ensuring monitoring needs are performed and
use the opportunity to support patients achieve better health outcomes through
shared decision-making conversations Support patients to take their medications
to get the best from them, reduce waste and promote self-care
Make appropriate
recommendations to prescribers
Promote safe
disposal of medicines and recycling of inhaler
Contribute to the
safety of drug monitoring processes and drug alerts
Analyse,
interpret and present medicines data to highlight issues and risks to support
decision making
Adhere to
organisational policies and procedures, including confidentiality,
safeguarding, lone working, information governance, and health and safety.
Duties will vary
according to the demands on the practice as agreed with the practice Clinical
team.

Goldsworth Medical Practice is looking for a Clinical Pharmacist with experience of working in a GP Practice in the UK to join our friendly, welcoming, and innovative GP Practice.
We are located in Woking and delivering care to over 13,000 patients, achieving high QOF performance and maintaining a Good CQC-rated service. We are seeking an experienced, motivated and forward-thinking Clinical Pharmacist who has worked in General Practice.
We would love a pharmacist with a prescribing qualification but if they dont have one we would support further training.
We are a dynamic, young and exciting large team in General Practice. We have 1 pharmacists technician, 4 Nurses, 2 HCAs and 4 partners.

To reconcile medicines following discharge from hospitals or intermediate care and
working with patients and community pharmacists to ensure patients receive the
medicines they need post-discharge and to reduce risk of readmission.
To be involved in multidisciplinary clinical meetings within the PCN where
appropriate
Contribute to the provision of out of hours/on call/extended services for the
practice and the patients. These can include patient facing and telephone
consultations, signposting to other services and/or healthcare professionals where
appropriate, while working within scope of practice and limits of competency
To manage repeat prescription requests, resolving queries where possible within
scope of practice. To ensure patients have appropriate monitoring tests in place
when required.
Implement changes to medicines in line with MHRA alerts, product withdrawal or
shortage and other local or national guidance
To maintain full and complete records of all patient contacts using appropriate
clinical templates and coding
To undertake a proactive role in audit and quality improvement implementing
recommendations where appropriate.
Provide independent information to patients with regard to medicines and
prescribing changes, initiating further support from other healthcare professionals
where appropriate
Participate in review and setting of policies relating to medicines management and
prescribing and help ensure practices prescribe in accordance with local guidelines
and formulary
To identify areas of clinical risk at network level and make recommendations to
support the introduction of new working practices that will optimise the quality of
prescribing and make more efficient use of network resources.
To monitor network practices prescribing against the local health economy and
make recommendations for GPs or amend within scope of practice, based on
findings.
To maintain own clinical and professional competence and be responsible for own
