Practice Nurse with training in Asthma and COPD

Job summary

We are seeking a dedicated and proactive Asthma & COPD Nurse to join our supportive team at Blackwater Medical Centre, situated in the beautiful and historic town of Maldon, Essex, with excellent access to the A12.

We are a busy and well-organised practice serving around 13,000 patients, supported by a multidisciplinary team including

GPs

Nurse Practitioners

Practice Nurses

Clinical Pharmacists

Physician Associate

Care Co-ordinator

Social Prescriber

We also benefit from an on-site dispensary and a community pharmacy that is open 7 days a week, enabling us to provide streamlined care for our patients.

Main duties of the job

Role Responsibilities include:

General nursing duties with a focus on asthma and COPD management

Reviewing and managing patients with respiratory conditions in line with current guidelines

Supporting QOF targets and helping the practice maintain high-quality outcomes

Working collaboratively with GPs and the wider team to ensure excellent patient care
