Job summary
This is a new role for SWAGGA PCN so were looking for someone who can work as part of a team to identify needs and make improvements in the way care is managed for elderly patients. Youll need to be a strong communicator with good administration skills but most of all, were looking for people with passion and compassion.
As a crucial member of the team, you will help us improve care in care homes and support our member practices in their care of frail patients. In return, youll receive training and support and will gain extensive knowledge about NHS care planning, and healthcare systems and processes.
Main duties of the job
This post will be to work within a Care Homes & Frailty Team, with Care Homes being geographically aligned to each team member. However, covering other Care Homes during team member absences will also be part of the role.
A good working relationship with the Care Home Manager will be paramount to the role and you are expected to develop this quickly.
In addition, key requirements of the role include:
Collecting MUST scores and FRAT scores that will need to be entered on Emis WEB for the patients GP to receive this information (training will be provided).
Audit information as requested by clinical leads.
Information from ward rounds will need fully collating onto Emis WEB to ensure the patients care record is kept fully up to date.
Liaising with GPs on behalf of Care Homes when required.
Organising MDT Meetings.
Feeding back key data and information to the wider PCN leadership team.
About us
Our network covers 15 practices in South Liverpool. Our combined patient population is approximately 101,500 and we work very closely with our community team and other local Healthcare providers. This includes Salaried GP's, Advanced Clinical Practitioners, Clinical Pharmacists, Practice Nurses, Social Prescribers, First Contact Physios and Physicians Associates.
The network has a number of care homes (residential and nursing or joint) with capacity to provide care for nearly 600 residents. This includes homes ranging in size from 27 60 beds and some supporting those living with dementia and /or complex physical health needs. You will work as part of our local multi-disciplinary team (MDT) to ensure that all those living in care homes are able to live and age well and, when the time comes, to meet their end of life care wishes. You will build strong and effective relationships with our care home leads and act as a point of contact for residents, families and professionals who visit care homes, such as MDT members and in-reach specialists. As a result, you will be working with health and care providers across a range of services, including Medicines Management Teams, and Community and Therapy Services. You will also take part in ward rounds (often in person), to support the MDT.
