We have an opportunity for a part-time
Practice Nurse to join our efficient, developed and well-supported nursing
team. If you are enthusiastic, caring and passionate about delivering the
highest quality nursing care, keeping patients at the heart of all you do, we
want to hear from you. The post will be delivered over three days, between 8.30am and 5.30pm on a Tuesday, Wednesday and Friday (Wednesday is negotiable).
This is an excellent opportunity to join an

improving our community's health.
The Practice uses EmisWeb with Docman and is
one of four Practices within the Whitfield Primary Care Network.
Main duties of the job
You will work collaboratively as part of our
multidisciplinary team of clinicians to meet the needs of our patients;
delivering the highest standards of patient care. You will undertake a
diverse range of nursing assessments and provide the appropriate care /
treatment according to patient needs, striving to improve standards of
care and enhancing nursing services within the practice.
About us
The Practice has a list size of 8150. The clinical team comprises of five GPs, two Nurse Practitioners, a Nursing Associate, a GP Assistant and two Clinical Pharmacists. We also have the support from a shared PCN workforce including Social Prescriber, Clinical Pharmacy Team and First Contact Physiotherapist.
We have an excellent management and adminstrative team with workflow direction and care navigation embedded in practice.
We have full QOF achievement and are a training Practice.

- Contribute
to the development of policies / protocols / guidelines and Standard Operating
Procedures and work within them at all times. Escalate issues as appropriate if
these standards are not identified and followed.
- Support
the implementation of evidence-based guidelines in conjunction with the
practice nursing team.
- Treatment
room skills, simple wound dressings, suture and clip removal.
- Family
planning.
- To
run independent clinics in surgery and in patients homes or nursing/residential
care setting to deliver holistic care for a variety of Long Term Conditions
such as Asthma, COPD, Diabetes, Coronary Heart Disease, Hypertension
monitoring, Anticoagulation.
- To
perform phlebotomy and administer vaccinations to children and adults.
- Perform
ECGs as part of the diagnosis and monitoring criteria.
- Provide
chaperone assistance if required.
- Flexibility
to take on extra hours/change shifts during annual leave or during periods of
absence.
- Maintains
high standards of infection prevention and control at all times thus protecting
patients, self and colleagues.

engage patients and carers in the management of their health, promoting health

appropriate waste is managed, segregated, contained and stored.

all consultations and treatments are documented accurately, concise, legible
and recorded in the patients notes at all times.

attend and participate in practice meetings when required.

communication skills and basic computer skills are required.
