Job summary

Fleetwood PCN are looking to recruit a Digital and Transformation Lead to join their team. As Digital and Transformation lead you would be expected to support increased access to care for patients, by supporting the adoption and/or optimization of new technology and other initiatives to improve the care offer and enabling PCN staff to work more effectively to support the sustainability of general practice services.

You will work collaboratively with other members of the PCN leadership team.

Main duties of the job

a. Improve adoption and/or optimisation of new technology to enhance patient access and experience and increase PCN productivity.

b. Build relationships and facilitate collaboration between practices and the wider system to support the delivery of care to patients (including shared appointments between practices to aid delivery of enhanced access)

c. Lead an improvement approach to change including building capability for quality improvement within the PCN and system wide approaches to problem solving

d. Review and improve the PCNs digital maturity e. Use data, and improve data quality, to: understand demand, capacity and activity hit government set targets Digital & Transformation Drive improvements in patient experiences of access and staff experience at work. support population health management support understanding of the type and intensity of support/training needs of the PCN and coordinate this support.

Support organisational change and the uptake of initiative that support and embed excellence within the health system. Manage the set up and delivery of multiple projects within a programme of work, working with colleagues and subject matter experts to scope, plan design and deliver those projects. Review project reports and risk logs for projects, advising individual project managers on escalation process and supporting delivery of projects where there may be significant barriers to delivery. Manage the whole lifecycle of a programme of work containing multiple projects, reporting on progress and managing risks and issues, escalating where necessary to the project board. Provide expertise of best practice change and project management methodologies. Ensure realization of benefits for customers through planning and designing and that project outputs meet programme requirements. Support organisational change and the uptake of initiatives that support excellence. Demonstrate flexibility and support new programmes and projects where necessary.
