Job summary
The role is to deliver a Social Prescribing Service to the patients of the Royal Docks medical Practice .
To implement patient referrals from participating GPs and other primary care
health professionals, providing holistic assessments, co-designing a social prescription to
improve health and well-being outcomes for individuals with a longer term outcome of
reducing the number of clinical /medical interventions required.
Primarily based within the GP practice social prescribers enable people to manage and improve their health,
wellbeing and resilience by connecting them into community services, groups, activities and
other pursuits.
Main duties of the job
Social prescribing helps to strengthen personal and community resilience, and reduce health inequalities by
addressing the wider determinants of health, such as debt, poor housing and physical inactivity.
The Social Prescriber would provide holistic support, connecting people to local
initiatives and services for practical and emotional support. The service aims to increase peoples
connection with others and their local community and empower them to take control of their own health and
wellbeing.
The Social Prescribers role will focus on urgent referrals into the service and proactive wellbeing calls to patients
whom are in need of rapid and holistic support.Direct referrals to support services such as debt, welfare benefits, digital literacy, and physical
activity

The Royal Docks Medical Practice is a well-established GP practice. We currently have 1 GP Partner and 8 salaried GPs
The practice a growing list size of over 12000 patients
The practice provides services from a modern, spacious, purpose-built premises.
The Practice is forward thinking and adaptable but runs a traditional list-based system with a strong ethos of continuity of care.
Working alongside the GPs is a wide-ranging team including 2 Practice nurses Physiotherapist , Healthcare Assistant and a large Administrative Support Team Care-Cordinator Socia& Wellbeing coach
The Royal Docks Medical Practice is a training practice currently hosting both GP Trainees and Medical Students.
Also, within the medical centre we are very fortunate to have our patients seen by the community Midwifery team.
As a general philosophy the aim of the clinical and admin staff is to preserve the best elements of a traditional style of personal, friendly, accessible family medical practice based on teamwork, which can react quickly and sensitively to daily demands, with well-equipped premises and efficient systems for prevention and planned care of chronic conditions. The practice provides a comprehensive range of Primary Care services to its patients and also provides GP services to a local forensic psychiatric hospital.
The success of the practice is also evidenced by its ability to attract new patients.

work with patients, using behaviour techniques such as motivational
interviewing to support them to identify, set and meet achievable goals to
improve their physical and emotional wellbeing.
- To
work with patients in our surgery, to signpost them to local
services/organisations which the patient themselves feel would be of benefit to
reach their goals. These services may include, befriending, weight management,
mental health support, smoking cessation, local social groups, leisure centres
and volunteering.
Demand for healthcare is increasing and cost pressures on the NHS in Social Prescribing (SP) is a way to link patients with other non-medicalised sources of community support such as gardening, well-being groups. Community gardening has a range of psychological benefits through access to nature, and nurture through community group participation. It also provides activities that increase levels of physical activity and access to fresh fruit and vegetables. As such community gardening delivers a holistic approach to supporting patients physical, mental health and wellbeing.
To support and work with the practices Patient Participation Group.
To
work with partner organisations to support the development and delivery of an
integrated care plan for the client so all their needs are met in a holistic way.
To
follow up with patients, update their records and support them on an ongoing basis.
To make and maintain
contact with local social prescribing providers and build a network and knowledge of referral routes
to and from service providers
To
participate in the Practices frailty multidisciplinary team (MDT) meetings on
a regular basis, and safeguarding processes when appropriate
To
undertake regular training to extend and update own knowledge of health and
wellbeing issues.
