Job summary

The Ashton Primary Care Network is seeking an experienced Paramedic to join our multidisciplinary team and support the delivery of high quality, patient centred care across our practices. This role is ideal for a paramedic who has already worked within primary care working autonomously but with debriefing and clinical support available from a GP, and is skilled in delivering both clinic based assessments and home visits.

Main duties of the job

  • Provide same day and routine bookable clinics, assessing, diagnosing and managing patients presenting with undifferentiated, undiagnosed, or acute conditions.

  • Undertake home visits for acutely unwell, frail, or housebound patients, carrying out full clinical assessments and escalating appropriately.

  • Work autonomously within your scope of practice, with access to GP debrief, advice and clinical support when required.

  • Perform clinical examinations, formulate differential diagnoses, and develop safe, evidence based management plans.

  • Initiate treatment, including prescribing (if qualified), ordering investigations, and arranging followup or referral to appropriate services.

  • Support longterm condition management where appropriate, contributing to proactive care and reducing avoidable hospital admissions.

  • Provide effective safety netting, patient education and self management advice.

  • Document all consultations accurately using the PCNs clinical system (EMIS), ensuring appropriate coding and contribution to QOF/PCN targets.

  • Work collaboratively with GPs, ANPs, pharmacists, social prescribers, care coordinators and other MDT members to ensure continuity and high quality care.

  • Participate in MDT meetings, case discussions, clinical supervision and quality improvement activities across the PCN.

  • Adhere to safeguarding responsibilities, professional standards and PCN governance frameworks.
