Social Prescribing Link Worker

Job summary

Social Prescribing Link Workers offer a unique, non-medical approach to enable our patients to manage and improve their health and wellbeing. An opportunity has arisen to employ two full-time Social Prescribing Link Workers to work on behalf of all 8 practices within the Ashton Primary Care Network.

Following referral from General Practice, you will be responsible for an initial patient assessment and introducing links to services, agencies and organisations who are best placed to help improve health and wellbeing.

Main duties of the job

The successful candidates will be able to demonstrate the ability to be a good listener, have time for people and be committed to supporting local communities to care for each other. You have have experience of working positively with people facing complex social and emotional challenges. You will have great interpersonal skills and a can do approach to supporting people, community groups and local organisations. You will have knowledge of the local area and groups available and the ability to navigate a complex directory of services across voluntary, community and public sector organisations.

About us

The role is based in Ashton-Under-Lyne receiving referrals from the 8 different practices who have a cohesive working relationship. The role is office based and may include seeing patients face-to-face in various locations within the Community, a driving licence and access to a car is therefore essential.

The role will involve liaising with various roles within our PCN including Care Coordinators, Clinical Practitioners, Pharmacists, Paramedics, Physician Associates and General Practice staff.
