Job summary
Dr Arora's Surgery are looking for a practice nurse with an interest in diabetes to join our healthcare team to support our patients with type 1 and 2 diabetes.
Previous applicants need not apply.
We require a minimum of 16 hours per week, however we have the flexibility to increase for the right candidate.
The main duties are to run diabetes clinics, supporting patients, family members, carers and other healthcare professionals with the aim of empowering patients to self manage their own diabetes, as well as other general practice nursing duties.
Experience in advanced therapies such as GLP-1's, SGLT-2's and insulin desired. Independent prescribing qualification would be an advantage, however this is not essential.
Main duties of the job
- Provide a first point of contact within the practice for patients living with diabetes and non-diabetic hyperglycaemia, making use of skills in history taking, physical examination, problem-solving and clinical decision making, to establish and review management plans.
- Make professionally autonomous decisions for which he/she is accountable.
- Instigate necessary diagnostic tests or investigation and interpret findings/reports.
- Provide safe, evidence-based, cost effective, individualised patient care.
- Offer a holistic service to patients and their families, developing where appropriate an on-going plan of care/support, with an emphasis on prevention and self-care.
- Promote health and well-being through the use of health promotion, health education, screening and therapeutic communication skills.
- Refer patients directly to other services/agencies as appropriate.
- Work with multi-disciplinary teams within the practice to promote integrated care.
- Contribute to the practice achieving local and national targets to sustain the high standards of patient care and service delivery.
- Other practice nursing duties as required such as childhood immunisations, cervical screening and travel immunisations.
