Job summary
The successful candidate will be responsible for a number of clinical
areas such as chronic disease management, health promotion, Immunisation,
Cervical screening as well as actively supporting the practice management team
in the reviewing and delivery of clinical policy and procedure.
A training plan will be provided for any Nurses wishing to expand their knowledge into Practice Nursing.
Main duties of the job
a. Assess the needs of patients ensuring the provision of healthcare is
appropriate, incorporating evidence-based practice
b. Health
promotion and wellbeing programmes
c. Implement and evaluate individual treatment plans for chronic disease patients
that promote health and wellbeing
e. Provide routine nursing care to patients in accordance with
clinical based evidence, NICE and the NSF
f. Manage
chronic conditions

the provision of travel medicine services

Phlebotomy when required
k. Chronic Disease & referrals
l.
Maintain accurate clinical records
m. To ensure read codes are used
effectively

To
prioritise health issues and intervene appropriately

To
support the team in dealing with clinical emergencies
t. Implementation of vaccination programmes for adults and children

To
liaise with external services/agencies to ensure the patient is supported

Cwmbran Village Surgery situated in Cwmbran, Torfaen, Wales - nestled on the Monmouthshire-Brecon Canal, just North of the city of Newport. The practice is an easy drive from Cardiff (30mins) and North Bristol (35mins), there are also excellent rail links with regular services to Newport, Cardiff and to the North. The following are the core tasks of the Trainee Practice
Nurse. There may be, on occasion, a requirement to carry out other tasks. This
will be dependent upon factors such as workload and staffing levels:
