Job summary

We have a vacancy for a Coder and Summariser, due to retirement, for 20 hours per week over 4 days.

We are looking for a well motivated, enthusiastic applicant to join our proactive team.

Main duties of the job

The main duties include:

  • Adding information to patients' records from hospital letters, discharge letters or other correspondence received, via EMIS Web
  • Actioning all incoming Out Of Hours reports that come in on a daily basis
  • Summarising incoming notes or updating historical information within patients records

There will also be other data entry work or data entry checking included in the role.

Our current Practice Team consists of the following:

  • 2 GP Partners
  • Salaried GP
  • 2 Nurse Practitioner
  • 2 Practice Nurses
  • Health Care Support Worker
  • Practice Care Facilitator

Non- Clinical Team:

  • Practice Manager
  • Business Manager
  • 2 Office Managers
  • 15 Reception & Admin Staff

We are also a committed training practice. We currently have a GP Registrar and Foundation Year 2 Doctor on placements at the practice.

In addition to the practice-based team, as a member practice of Newcastle North Primary Care Network, our practice team and patients benefit from the support of the wider PCN team. This includes Pharmacists, Pharmacy Technicians, Dieticians, Mental Health Practitioners, First Contact Physiotherapist, Paramedic, Care Home Team, Social Prescribers, Health and Wellbeing Coach and Care Coordinators.

Job responsibilities

Key areas:

  • Work as a member of our coding and summarising
  • Inputting coded data into EMIS Web (clinical system) from clinical letters, discharge letters and variety of other sources
  • Maintain clinical coding skills and participate in training programmes as required to work towards a high standard of coding
  • Extract, interpret and independently code clinical information from clinical letters and discharge summaries from hospitals and other organisations
  • Summarise existing and new patient records to a high standard and in accordance with practice policies and procedures
  • Assist practice colleagues with queries relating to medical records
  • Run searches and assisting in data cleansing when required or instructed- This includes participating in quality improvement and indentifying areas where coding processes may be improved.
  • Communicate effectively with patients, carers and other agencies when required through their employment
  • Maintain effective communication within the Practice team and attend appropriate scheduled meetings e.g. Practice and coding meetings
  • Use appropriate infection control procedures, maintaining work areas in a tidy and safe way

Information that relates to staff, patients, carers or the business may only be divulged to authorised persons in accordance to practice policies or procedures that relate to confidentiality and protection of personal/ sensitive data.

  • GCSE Grades in English and maths or equivalent qualification.
  • Clinical Readcode training e.g. Snomed or Emis.
  • Experience of summarising records.
  • Experience of working in a Health Care setting.
  • Excellent communications skills (written and oral).
  • Strong IT skills.
  • Ability to prioritise and work to deadlines.
  • Effective time management.
  • Polite and confident.
  • Motivated and pro-active.
