Job summary

An exciting opportunity has arisen for an experienced Clinical Pharmacist to undertake a pioneering role within primary care.

APEX PCN is currently taking part in the NHSE national pilot to consider a proactive approach to meeting our patient needs. As part of this programme we are recruiting a fixed term Complex Care support team for our PCN population. This team will be assigned as a micro team to serve individual patient needs and take a consistent and personalised approach to patient care.

This is a hugely exciting opportunity for us to work closely with our identified patients through a proactive MDT approach.

The candidate will be available for Hybrid working, with a need to be patient facing and interacting with the wider team.

Main duties of the job

To work as part of a multi-disciplinary team in a patient-facing role.

The Experienced Clinical Pharmacist will play a pivotal role in providing comprehensive pharmaceutical care to a complex patient cohort, focusing on proactive strategies to optimise medication therapy, reduce risks associated with polypharmacy and inappropriate prescribing, and improve overall patient outcomes. This role involves utilising advanced clinical skills, including independent prescribing, to manage long-term conditions, minimize unplanned hospital admissions, and enhance medicines safety. A key aspect of this role is conducting Comprehensive Geriatric Assessments (CGA) and holistic needs assessments to ensure a patient-centred approach. The pharmacist contribute to the pilots quality improvement activities, ensuring adherence to local and national guidelines.

Previous experience of working in a GP practice is desirable but not essential. Qualifications

Essential

  • Master's degree in Pharmacy (MPharm) or equivalent.
  • Registered with the General Pharmaceutical Council (GPhC).
  • Independent Prescribing qualification.

Experience

  • Significant experience working autonomously in a clinical setting, preferably within primary care.
  • Demonstrable experience in managing complex patient cases and conducting comprehensive medication reviews.
  • Specialist knowledge acquired through post-graduate diploma level or equivalent training/ experience
  • Commitment to continuous professional development and staying up-to-date with the latest clinical guidelines and research.
  • An appreciation of the nature of GPs and general practices.
  • An appreciation of the nature of primary care prescribing, concepts of rational prescribing and strategies for improving prescribing.
  • Is able to plan, manage, monitor, advise and review general medicine optimisation issues in core areas for long term conditions.
  • Able to obtain and analyse complex technical information.

Desirable

  • Experience with deprescribing initiatives.
  • Knowledge of strategies to reduce falls and hospital admissions etc.
