Job summary

An exciting opportunity has arisen at Belgrave Medical centre for a experienced Advanced Nurse Practitioner to join the nursing team in our busy surgery

Hours of work will be 37.5 hours over 5 days and the hours will be between 8am - 6.30pm

The role will involve providing direct access clinics, undertaking an initial assessment and treatment. The ideal candidate must have a good understanding of general practice.

A caring yet firm and calm approach is vital as is excellent communication skills and a passion for improving services for patients. Strong communication and clinical skills are essential. You should have strong IT Skills with experience of Emis Web

Main duties of the job

To initiate assessment, diagnosis and the implementation of a range of care interventions for patients presenting with undifferentiated and undiagnosed problems.

To evaluate the effectiveness of the treatment and referrals through autonomous and accountable decisions in nursing practice supported by current research and in accordance to Practice protocols and procedures.

About us

We are a welcoming, friendly team based in Longton in Stoke-on -Trent.

We currently have 15600 patients. Our clinical team consists of 3 GP Partners, 3 Salaried GP , 3 Advanced Nurse Practitioners, 5 Practice Nurses, 1 Healthcare Assistant and a Phlebotomist. We have 1 Practice Manager, 1 Assistant Practice Manager,1 reception manager and admin team of 16.

We are also a training Practice. All of our employees are hard-working and we all strive to deliver excellent patient care at all times.

Job responsibilities

Main Duties and Responsibilities

To write, and maintain policies within the practice for clinical areas and to oversee the development of policies written by the practice nurses

To promote best practice in the implementation of such policies

Identify, monitor and review deficiencies in procedures and implement remedial action

To work with colleagues to ensure contractual targets are met

Provide a first point of contact in the practice for patients presenting with undifferentiated, undiagnosed problems, making use of skills in history taking, physical examination, problem-solving and clinical decision making to establish a diagnosis and management plan

Instigate necessary diagnostic tests or investigations and interpret findings/reports

Instigate radiology requests ensuring training in accordance with the local hospital trust

Monitor indicators of chronic disease anticipating possible decline and proactively manage to enhance well-being and maintain independence

Prescribe medication and review medication for therapeutic effectiveness

Develop evidence based specialised care plans for patients

Plan interventions in light of patients expressed wishes and goals of care

Evaluate possible treatment options as diseases progress

Refer patients directly to other services/agencies as appropriate

Admit patients directly to secondary care hospitals in acute medical need

Communicating effectively with patients and carers, and recognising their needs for alternative methods of communication

Provide expert advice to patients

Undertake consultations to a high standard in the surgery, over the telephone and in the patients own home

Record accurate consultation data in patients records in accordance with the latest NMC guidance and other pertinent standards

Know the general principles of first aid and resuscitation to be able to undertake initial actions as appropriate

Provide teaching to e.g. existing staff, HCAs, practice nurses, nurse practitioners, medical students, GP registrars

Provide leadership and advice to the nurse practitioner, practice nurses and healthcare assistant supporting them to develop and maintain their clinical skills

Carry out audits and research as required and monitor own practice

Attend and participate in any Practice meetings

Develop the nursing service provided by the practice

Help develop and set up new patient services and participate in initiatives to improve existing patient services
