Job summary
Stonyhill Medical Practice are looking for an enthusiastic Clinical Pharmacist to join our friendly and supportive team.
The role is part-time (24 hours per week).
This exciting role will offer the successful candidate the opportunity to work alongside a multidisciplinary primary care team to deliver exceptional care to 12,500 patients.
The role will put to use a wide variety of skills in delivering direct clinical care to patients and playing a key role in the overall medicines management of the practice.
Working hours and salary will depend on the ideal candidate.
Main duties of the job
To be responsible for implementing an effective medicine management system within the practice, including cost-effective prescribing, providing advice and support for both patients and colleagues. In addition, the post-holder will also support the multi-disciplinary team, in line with the strategic objectives of the practice. You will work closely with the Senior Management Team and help with developing and implementing processes and developing the team.

Our clinical team comprises of 6 GP Partners, 2 Salaried GPs, ANPs, Paramedic, Practice Nurses, Clinical Pharmacists, Practice Nurses, Nursing Associates, HCAs, Health & Wellbeing, Mental Health Nurse, MSK Practitioner and a large administration team.

Job responsibilities
Monitoring, advising and optimising treatment for patients in relation to their long-term conditions (eg diabetes, asthma, hypertension, COPD)
Managing and authorising repeat prescriptions
Conducting thorough medication reviews
Providing patients with advice and guidance on the effective use of medicines
Providing patients with guidance and treatment in respect of their blood results in line with practice protocols
Offer patients appropriate advice and treatment in line with NICE guidance in respect of their acute or on-going medical needs (as per practice protocols and based on the individuals clinical skills and knowledge, eg minor illnesses, contraception, anticoagulation etc)
Utilising their clinical judgement to assess acute prescription requests
Diligently processing medication changes from discharge notes and outpatient letters
Developing and improving practice standards in relation to medicines management, eg conducting audits, producing protocols and providing guidance to the wider practice team
