Job summary
Longtown Medical Practice is looking for an experienced nurse to join our team and offer excellent,
quality care to our patients. Ideally
you will be trained in the management of one or more chronic diseases (preferably diabetes) although
training may be available for the right candidate.
Salary is dependant on experience.
Main duties of the job
Our Practice Nurses are expected to provide and maintain a high standard of nursing care for our patients as well
as providing nursing assistance to GPs and other members of the practice
and primary care team.
They will assess,
plan, develop, implement and evaluate treatment plans that promote health
and wellbeing, and for patients with long term conditions. They will proactively
identify, help in diagnosing and manage treatment plans for patients at
risk of developing a long term condition.
Other duties will include:
- Medication reviews
- Supporting patients to adopt health promotion strategies that promote patients to
live healthy, and apply principles of self-care
- Working with patients presenting with mental health needs in
accordance with the NSF for Mental Health - to be aware of local mental health care pathways, and
referral/support systems.
- Implementing and participating in vaccination and immunisation programmes for both adults
and children
- Meeting the needs of patients presenting with opportunistic wound care.
Where the post holder has completed an approved prescribing qualification they may be asked to prescribe safe, effective and appropriate medication as defined by current legislative framework and within personal competence and agreed formulary boundaries.

Key clinical responsibilities:
