Job summary
A small, semi-rural GP practice, we are looking for an experienced Advanced Nurse Practitioner to join our team
and offer excellent, quality care to our patients.
Main duties of the job
The successful applicant will have a key role in developing new services
in the practice and collaborating with other providers to develop innovative
ways of working in the future. You will
be involved in managing urgent/on-the-day requests for appointments, visits,
telephone consultations, enquiries and queries as well as the opportunity to
establish your own caseload and special interest. Applicants who are motivated,

About us
Longtown Medical Centre is a small friendly practice based in Longtown in Cumbria. We have a practice population of around 3800 and an experienced and committed team of GPs, Nurse Practitioner, Practice Nurses, HCA and Frailty Coordinator supported by our small but highly effective Reception and Administration teams.
We can offer a competitive salary, flexible
working hours, a purpose built facility where you can work with other Community
Healthcare Professionals and excellent GP support. We are flexible in relation
to hours but would ideally like someone to work up to 2 days per week. We would consider job share. Salary will be based on experience.

The post holder will be an experienced nurse who, acting within their professional boundaries, will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of their care. They will demonstrate safe, clinical decision-making and expert care for patients within the general practice. They will work collaboratively with the multi-disciplinary general practice team to meet the needs of patients, supporting the delivery of policy and procedures, and providing nurse leadership as required. In order to work at this level, NMC requirements for advanced practice must be met.

- Registered General Nurse (Currently registered with the Nursing & Midwifery Council)
- Recognised NP qualification at minimum of degree level or equivalent
- Independent Non medical prescriber (or willing to train)

- Teaching / Mentoring experience and /or qualification

- Minimum of 5 years post registration experience
- Demonstrate clinical Knowledge in acute and chronic disease management
- Demonstrate clinical competence in minor illness management
- Evidence of appropriate continuing professional development activity to maintain up-to-date knowledge and on-going competence in all aspects of the NP role
- Evidence of working autonomously and as part of a team
- Ability to undertake home visits.
