Job summary
This is a great opportunity to join an experienced team as well as have access to national training through the NHSE Clinical training pathway. We work closely across our practices, to support our healthcare professional team in safe and effective patient care. Pharmacy Technician will support the other Technicians and Clinical Pharmacists to deliver improvements to maximise safe, cost effective best practice in prescribing and be an integral part of a great patient experience.
The post holder will help patients to get the best from their medicines through effective medicines reconciliation after discharge and on other transfers of care, medication use reviews, identification of patients suitable for electronic repeat dispensing, promotion of online ordering, providing assurance of monitoring of high risk drugs, and handling of prescription queries within the areas of competence.
In addition, the post holder will be responsible for encouraging the development of better understanding of the principles of medicines optimisation throughout the practice teams and promoting good practice in line with therapeutic developments. This will involve assisting the PCN in achieving national requirements, NICE implementation and utilisation of medicines optimisation initiatives.
Main duties of the job
- support medication reviews and medicines reconciliation for patient transfers between care settings
- dose optimisation and medicines synchronisation.
- Improve prescribing processes in general practice.
- Promotion of electronic repeat dispensing and online ordering
- Liaise with other pharmacy stakeholders e.g. CCG/ICB/ICS Medicines Optimisation team, community pharmacy, and hospital pharmacy teams
- Handle function specific information, which may be sensitive, complex or confidential and appropriately recording, transferring and/or coordinating such information in accordance with the Data Protection Act; Caldicott Guidelines and the Confidentiality Code of Conduct
- To be responsible for the organisation, planning and of own workload to meet set deadlines
- To assist in the monitoring of prescribing expenditure at individual practice level and propose changes to ensure containment within identified resource
- To undertake learning and development to ensure the required knowledge and skills for practice-based work

Mythe Medical Practice, the host and base practice for this role, is a CQC Outstanding practice and a hub for innovation. The practice is based in the Devereux Centre, with excellent staff facilities and is just a short walk from the beautiful town and the Abbey, riverside walks.
