Job summary

Clinical pharmacists have a key role in supporting delivery of the Network Contract DES Service Specifications, the Investment and Impact Fund (IIF) and local enhanced services. Our Pharmacists are an integral part of our team, having constant dialogue with the GP partners and all members of the team. For example, the Structured Medications Review (SMR) and Optimisation requirements tackling over-medication of patients, including inappropriate use of antibiotics, withdrawing medicines no longer needed through NHS-led programmes such as low priority prescribing and medicines optimisation more widely. For Enhanced Health in Care Homes residents will benefit from regular clinical-pharmacy led medicines reviews.

Clinical pharmacists will have a leadership role in supporting further integration of general practice with the wider healthcare teams (including community and hospital pharmacy) to help improve patient outcomes, ensure better access to healthcare and help manage general practice workload. The role has the potential to significantly improve quality of care and safety for patients.

Main duties of the job

Clinical Pharmacists will:

  • Provide expertise in clinical medicines review and address public health and social needs of patients.
  • Reduce inappropriate polypharmacy and wasteful prescribing through structured medication reviews.
  • Reconcile medicines following hospital discharge and work with patients and community pharmacists to ensure patients receive the medicines they need post-discharge.
  • Interface with community and hospital pharmacy colleagues and develop referral processes between primary care professionals including the promotion of the repeat dispensing service.
  •  Oversee the repeat prescribing reauthorisation process, including reviewing requests for repeat prescriptions and medicines reaching review dates.
  • Manage patients and health care professionals medicine queries.
  • Manage practice formularies to improve the quality, safety and cost effectiveness of prescribing.
  • Implement drug withdrawals and alerts e.g. MHRA, aimed at improving medicines safety.
  • Work with commissioning organisations to comply with local medicines optimisation schemes and projects.
  • Provide medicines information and training to healthcare professionals and admin staff.
  • Act as a source of medicines information for Mythe Medical Practice and the PCN, contributing to PCN-wide discussions, meetings, policies and best practice.

Mythe Medical Practice and TWNS PCN is a collaboration of 5 GP practices across Tewkesbury, West Cheltenham, Newent and Staunton (TWNS) with a shared population of 50,000 patients.  Our pharmacy team is already over 9 WTE strong, and will increase as a result of this role. The team are collaborative and supportive, meeting regularly to support each other and with links across other neighbouring PCNs in Gloucestershire, this role is an opportunity to make a significant difference to patients in an environment of best practice and professional inclusivity.

Mythe Medical Practice, the host and base practice for this role, is a CQC Outstanding practice and a hub for innovation. The practice is based at the stunning Devereux Centre, with excellent staff facilities and is just a short walk from the beautiful town and the Abbey, riverside walks and a whole range of independent retailers.

  • Completion of an undergraduate degree in Pharmacy, and registration with the General Pharmaceutical Council; with a minimum of 2 years post graduate experience in pharmacy, as demonstrated within a practice portfolio;
  • Is enrolled on, or is willing to do, or has qualified from, an approved 18-month training pathway or equivalent that equips the Clinical Pharmacist to meet the responsibilities set out within this Job Description and the requirements of the PCN DES, and be able to practice and prescribe safely and effectively in a primary care setting (for example, the CPPE Clinical Pharmacist training pathway);

Desirable

  • Independent prescribing qualification is desirable but not essential.
