Part Time Salaried GP - Concept House and Sefton Road Surgery

2 part time and 2 full time salaried GPs

Nursing team includes Practice Nurse and HCA - currently recruiting for an Advanced Nurse Practitioner to join the team

F2 doctors and medical students

Cheery and welcoming long established team

Outward looking we have interests in commissioning, minor surgery and cancer services

Main duties of the job

The post holder will provide personal medical service for the population of the practice to an acceptable standard, taking part in the day to day activities of the practice including surgery and home visits with appropriate administration to ensure continuity and quality of care for the patients. The post holder will also be expected to work to deliver quality care via the QOF in line with the set criteria and to work at all times to promote and enhance care to patients through delivery against QOF and other appropriate targets.

About us

We are a well-established team on a 2 site GP practice with 6500 (weighted) list size in Bootle. The Principal GP is Dr David Goldberg alongside a team of 4 salaried GPs, 1 Practice Nurse and a HCA. We are a training practice and work with Fy2 doctors and medical students. Our team are closely involved with the Integrated Care Board and the PCN and we have active support from clinical pharmacists, social prescribers and first contact physios.

Job description

Job responsibilities

Clinical

To provide high quality personal medical services in the context of general practice to the patients registered with practice

To provide a full range of GMS services in line with the agreed requirements of the practice contract including the quality and outcomes framework

To provide appropriate care and services to patients in all age ranges and level of health care need

Torespond to medical/health problems presented by patients in the registered list including history taking, diagnosis, investigation, treatment and referral as appropriate

To provide on call and other services in practice as appropriate.

To promote the use of end of life strategies (GSF etc) to deliver high quality palliative care.

To provide management of long term conditions in line with national guidelines and pathways

To prescribe generically for patients as appropriate to need and if none generics utilised be able to provide appropriate clinical rationale working in partnership with medicines management colleagues.

To provide appropriate health promotion and preventative health care advice to all patients attending the practice.

Non-Clinical

To carry out appropriate administrative and other duties in line with the needs of GMS contract and QOF including referral to other services.

To maintain clinical records either written or computer based in line with professional code and requirements utilising appropriate READ and other coding systems.

To work with the IT/analysts to provide as required appropriate IT based and other performance information to monitor contract and service delivery.

To liaise with all relevant agencies and services to promote knowledge and awareness of services in practice and local community.

To work with colleagues to identify ways to ease the burden of demand.

To work with GP lead/s to support and enable the delivery of PBC plans and other service enhancements.

To work to promote use of choose and book and other key targets (cancer waits 18 week targets etc) within the practice.

To identify ways to promote opportunities for patient involvement in service development and evaluation.

To work in line with appropriate NHS policies and procedures including human resources, safeguarding children etc.

To be actively involved and engaged in practice and wider team meetings and forums to ensure the delivery of quality services, communication and developments.

Training & Education

To provide appropriate health education and advice to patients and carers to enable self management and choice.

To undertake and complete appraisal process on yearly basis

To undertake appropriate training to meet mandatory and personal educational needs and practice development needs.

To deliver practice development projects in the practice as agreed with practice staff and managers

To undertake Practice/Personal development in line with CPD objectives and record activity

To share and disseminate acquired knowledge with all practice members through in practice developmental sessions and SEA or critical incident reviews.

Clinical Governance/Research and Audit

To undertake appropriate audit in practice to identify clinical quality issues and markers and define processes to improve clinical outcomes

To take part in appropriate R&D and audit processes within the practice as required

To develop and implement as appropriate guidelines, policies and procedures as required

To utilise latest available evidence to shape and organise services to reflect needs of practice population.

Communication

To develop and maintain high levels of communication with patients and carers ensuring effective care delivery.

To develop and promote communication processes between all team members to enable good working practices.

To maintain communication processes with acute and other colleagues and partners in care to ensure care delivery

Person Specification

Qualifications

Essential

  • Medical degree. Membership of the Royal College of General Practitioners or equivalent in training if approved by the Royal College of General Practitioners. Certificate of MRCGP.
  • Registered GP with the GMC (included on the GP Register).
  • Included on GP Performers List.
  • Possesses the right to work in the UK.
  • Certificate of satisfactory immune status re: Hepatitis B and other relevant viruses.
  • Current General Practice experience with up-to-date GMC appraisal/revalidation.
  • Childhood Surveillance.
  • At least 6 months each of acute A&E, medical and surgical experience. Confident in Womens Health.
  • Demonstrable ability to relate well to challenging patients, carers, relatives and staff.
  • Experience of working with the socially deprived, nursing home, chronic medical conditions, mental health and HIV positive patients.
  • Full driving licence, with use of own car.

Desirable

  • Firsthand experience of developing services in a constantly changing environment.
  • Ability to demonstrate an understanding of private general practice and how best to dovetail this with NHS services.
  • Knowledge of adherence to the CQC Fundamental Standards.
  • Understanding of contracting, service delivery and activity monitoring.
