Job summary
Concept House and Sefton Road Surgery have an exciting opportunity for a
highly motivated and committed Advanced Nurse Practitioner to join our clinical
team for 20 hours per week.
We are looking for an ANP who is committed to providing complete and
professional care to our patients. The ideal candidate will be expected to care
for patients who present with undifferentiated problems, minor illness/ailments
and undertake home visits when required (training and support will be
provided). They will be expected to demonstrate an ability to provide high
quality, evidence based clinical care.
Main duties of the job
As
an Advanced Nurse Practitioner, you will be an experienced clinician who,
acting within your professional boundaries, this will include telephone triage,
initial history taking, clinical assessment, diagnosis, treatment and
evaluation of care. You will demonstrate safe, clinical decision making and
expert care, including assessment and diagnostic skills. You will demonstrate
critical thinking in the clinical decision-making process.
About us
We
are a well-established team on a 2 site GP practice with 6500 (weighted) list
size in Bootle (Sefton CCG). The Principal Gp is Dr David Goldberg and we have a team of 4 salaried
GPs, 1 Practice Nurse and are currently recruiting for a HCA. We are are training practice and often have Fy2 doctors and medical students. Our team are closely involved with
the ICB, the PCN and we have active support from
clinical pharmacists, social prescribers and first contact physios.

Clinical Care
- Act as Advanced Nurse Practitioner, demonstrating advanced clinical competence and a knowledge base beyond those associated with conventional nursing roles.
- Undertake consultations with patients as an autonomous practitioner and using own clinical judgement to diagnose, treat or refer patients
- Diagnose and manage both acute and chronic conditions, integrating both drug- and nondrug-
- based treatment methods into a management plan
- Make direct referrals to primary, secondary and social services within locally agreed pathways, guidance and protocols.
- Work with patients and their carers in order to support compliance with and adherence to prescribed treatments
- Ensure care plans are in place, including emergency health care plans following discussion with the patient and family members
- Ensure that patients receive high quality clinical care, delivered in a timely manner.
- Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs
and in accordance with evidence-based practice and national and practice protocols, within scope of practice
- Ensure complete and accurate documentation of each and every patient contact.
