Job summary

An opportunity for a Medicines Co-ordinator to join Worden Medical Centre. Working as a wider team member supporting the surgery staff, PCN Clinical Pharmacist and Medicines Optimisation Team to ensure delivery of safe, effective and efficient systems for medicines optimisation, repeat prescribing and reducing medicines waste.

Review and update the practice prescribing policies, to ensure the practice staff are aware of correct procedures that are safe, efficient and cost effective.

Assisting the practice to deliver the targets for the local enhanced service contracts and Quality and Outcomes (QOF) targets.

Contact for medication related queries from surgery staff, pharmacies,care homes and patients.

Main duties of the job

To identify and work closely with all healthcare professionals involved in medicines management in the practice.

To reduce waste, minimise risk and improve the repeat prescribing process.

To improve prescription security by introducing best practice guidelines

To be responsible for ensuring recall systems are in place, and working, for the correct monitoring of amber drugs and those included in the enhanced services of the GMS contract.

To undertake practice-based ICB project work under the direction of the Medicines Optimisation Team Lead to ensure cost savings on the prescribing budget

To undertake proactive housekeeping management of the repeat prescribing process in accordance with practice and ICB protocols

To deal with non-routine, non-clinical medication queries that requires follow up and problem-solving techniques.

To help the Medicines Optimisation Team with any searches, data collection and letter production required for prescribing changes.

Identify patients who would be suitable for electronic repeat dispensing and Medicines Use Reviews (MURs).

To ensure information from MURs is acted upon as appropriate.

Develop and implement medicines optimisation initiatives through the work of the practice e.g. repeat dispensing.

Cover the prescription order telephone line when required.

We play a vital role in managing chronic diseases such as diabetes, hypertension, respiratory and heart disease.

The practice has been based at West Paddock since 1993 and we work as team-based care including GPs, Advanced Nurse Practitioners, Practice Nurses, Healthcare Assistant, Nurse Associate, Medicines Co-ordinators, Administrators, Care Navigators and Practice Management.

Worden Medical Centre is part of Leyland Primary Care Network, who provide the Practice with a Paramedic, Pharmacist and Physiotherapist, we have support from the Lifestyle Improvement Team who provide non-medical social care interventions.
