We have an exciting opportunity for a hardworking, enthusiastic and innovative Practice Nurse with a keen passion for Asthma to join our surgery. We are a large , busy and dedicated GP Practice with a patient list size of 13,000 and we continue to grow. We support the training and development needs of all our staff in all areas. The successful applicant will work together with a fully supportive Nursing and HCA team and be supported by a large clinical team of Clinical Services Lead, GP Partners and Salaried GP's.
Main duties of the job
To be responsible for the delivery of practice nursing services, working
as part of the practice multidisciplinary team, delivering care within their
scope of practice to the entitled patient population. The practice nurse will work
in collaboration with the nursing team in a number of clinical areas such as infection prevention and control, health promotion, chronic disease management,
health prevention, well women clinics, as well as supporting the management
team in the reviewing of clinical policy and procedure.
About us
Worden Medical Centre has been an established surgery
serving the Leyland population of 13,100 patients for more than 30 years. The
practice premises were built in 1993 and we have had a full refurbishment in
2018.
We have 6 GP Partners and 3 salaried GPs who all deliver an
excellent and progressive service to our patient population. We currently train
Y4 and Y5 medical students, with a long term aim to train GP trainees and FY2
doctors. The GP team is supported by an innovative team of clinical staff and
administrators.
Clinical Team:Clinical Services Lead, 2
Advanced Nurse Practitioners, 3 Practice Nurses, 2 Health Care Assistants and
Pharmacy Technician.
Administrative Team: Practice Manager, Patient
Services Manager and an Admin team of 18 staff.
We are part of Leyland Primary Care Network. As part of the
network there are currently various additional roles working with all practices;
Care Home Lead, Clinical Pharmacists, First Contact Physiotherapist, Social
Prescribers, Pharmacy Technicians, Paramedics, Health and Wellbeing Coaches and
Care Co-ordinators.

dependent upon factors such as workload and staffing levels:

To provide routine nursing care to patients as
required in accordance with clinical based evidence, NICE and the NSF

To request pathology services as necessary

To process pathology results as required

To maintain accurate clinical records in
conjunction with extant legislation

To ensure clinical codes are used effectively

To chaperone patients where necessary

To assist GPs with minor surgery and vasectomy
when required
