Job summary
Coseley Medical Centre is looking for a Practice Nurse to join their team working along side the current team of Practice Nurses and HCA's. The post is for 25/30 hours per week , the pay is dependant upon experience.
Main duties of the job
The post holder will work in accordance with the NMC Code of Professional Conduct and scope of practice.
The applicant should be familiar with the QOF framework, carry out long term conditions and able to manage diabetic patients. be able to demonstrate ability and transfer knowledge in creating solutions to problems. Plan, organise and prioritise own work, activities and more complex tasks.
Be able to work as part of a team to manage patient care.

The practice has a list size of 7,200 patients. We are a friendly practice serving the coseley area , established over 100 years. We have diverse health care professionals working at the practice, including GP; ANP; Paramedics and physician associates.

Provide nursing treatments to patients in participation with general practitioners or independently to agreed protocols.
- Provide general and specific health screenings to the practice patients (within agreed protocols) with referral to general practitioners as necessary.
Maintain effective liaison with staff in the practice, with appropriate regard to confidentiality.
Management of Chronic Disease
- Identifying significant abnormalities
- Diagnosis, monitoring and development of individual management plans; agreeing these as appropriate with the patient and other health professionals
