We are looking for a Nurse to join our busy practice who shares our vision in delivering the best healthcare for patients.

This is a great opportunity for an experienced practice nurse or a nurse considering a move into general practice.

Main duties of the job

To provide a high quality care service based on the needs of the Practice population. Includes treatment, screening, preventative care and patient education.

To manage patients with chronic disease/ long term conditions

Undertake cytology screening as part of the national screening programme

Provide travel health advice and administer appropriate vaccinations

Deliver the national immunisation and vaccination schedule for both and children and adults

Undertake contraceptive reviews

Undertake health promotion to help educate patients

Undertake Venepuncture

Select, carry out and interpret results of specific tests

Undertake anticoagulation clinics to include interpretation of blood tests and dosing

Act as chaperone when required

About us

We are a long-established and highly regarded training practice operating from excellent surgery premises in Darfield and Wath Upon Dearne, good transport links only 10-15 minutes from the M1 and A1. Our list size is 12,000 patients.

We are looking for a committed Practice nurse to join our clinical team of 7 GPs, 1 Practices Nurse and 2 Health care Assistants

Job description

Job responsibilities

Job Purpose:

To provide a high quality care service based on the needs of the Practice population which includes treatment, screening, preventative care and patient education

Duties and Responsibilities

Clinical

1.To manage patients with chronic disease/ long term conditions

2.Undertake cytology screening as part of the national screening programme

3.Provide travel health advice and administer appropriate vaccinations

4.Deliver the national immunisation and vaccination schedule for both and children and adults

5.Undertake contraceptive reviews

6.Undertake health promotion to help educate patients

7.Undertake Venepuncture

10.Act as chaperone when required

11.To maintain accurate patient records and enter onto the computer using agreed Read Codes

12.Ensure accurate completion of all necessary documentation associated with patient health care

13.To collect data and participate in regular Practice audit and research projects

14.To contribute to the preparation of any Practice development plans

15.To maintain own annual registration with the NMC and adhere to Code of Professional Conduct

16.To maintain a safe and therapeutic environment for all PHCT staff, patients, carers and co-workers from other agencies. Ensure all statutory regulations (e.g. Health & Safety, COSHH, etc) are implemented and adhered to

17.To promote a professional approach at all times by attire and attitude to patients and staff both in and out of the Practice

18.To maintain own continuous professional development and ensure PDP kept up-to-date in line with the requirements of the Practice

19.Attend and participate in staff and Practice meetings and in-house clinical training sessions

20.Any other reasonable duties that may be required from time-to-time
