Job Summary:
Bath Independents PCN is seeking a dynamic and
autonomous First Contact Physiotherapist to join our existing FCP and multidisciplinary
team. As an FCP you will be the first point of contact for patients presenting
with musculoskeletal (MSK) conditions, providing expert assessment, diagnosis,
and management within our GP practices. This role is central to improving
patient access, reducing GP workload, and enhancing MSK care across our member
practices.
Main duties of the job
Key Responsibilities
Provide expert MSK
assessment and management at first contact, without prior GP referral.
Develop and implement
personalised treatment plans, including advice, education, and exercise
prescription.
Refer patients for
investigations or to secondary care services when clinically indicated.
Collaborate with GPs,
nurses, pharmacists, and other allied health professionals to ensure integrated
care.
Contribute to service
development and quality improvement initiatives within Bath Independents PCN.
Maintain accurate clinical
records using EMIS or other practice systems.
Support patient
self-management and promote physical activity and healthy lifestyle choices.
Participate in clinical
supervision, peer support, and CPD activities.
Engage with local
community services and voluntary sector organisations to support holistic care.

Network (PCN) is a group of 3 practices which incorporate, Batheaston,
Fairfield Park and Widcombe Surgeries. We deliver strong patient care to our
30,000-patient population working collaboratively as a team.
