Job summary
At Downend Health Group we are looking for a part-time Care
Co-ordinator to provide support to our patient groups, specifically those with
identified care needs. If you are compassionate, resilient and
dedicated to making change, this could be the role for you.
As a Care-Co-ordinator, you will be supported in an
established team and work closely with GPs and other primary care professionals
within the PCN. Your role will include ensuring support is made
available to our patients and their carers, and that their health needs are
addressed.
You will have excellent communication skills and understand
the need for the strictest confidentiality. We will provide role specific
training and support for the successful candidate.

Main duties of the job
The successful applicant will support patients in preparing
for or in following-up clinical conversations they have with primary care
professionals. The applicant will work closely with the GPs and other primary care
professionals within the PCN to identify and manage a caseload of identified
patients, making sure that appropriate support is made available to them and
their carers and ensuring that their changing needs are addressed.

Downend Health Group was formed in 2018 due to the merger of
two well established practices located in the suburbs of Bristol. The surgery
operates from two purpose-built premises located less than a mile apart.
As one of the largest teaching GP Practices in Bristol,
Downend Health Group (DHG) cares for over 27,000 patients and this number
continues to rise each year.
This excellent facility offers patients a family friendly
service with efficient and easy access to a highly trained team of health care
professionals. The medical team comprises 11 Partners (9 GPs, and 2
Pharmacists), over 20Salaried GPs, Clinical Pharmacists, Paramedics and a dedicated team of Nurses and Healthcare Assistants. We are supported by our experienced Care Co-ordinator Team, Medicines Management Team, Administration Team and Reception Team who provide excellent support both front of house and behind the scenes.
The surgery provides modern healthcare services and both
buildings offer calm, welcoming environment for patients, with enhanced and
pleasant working conditions for clinicians and staff.
We use EMIS Web and electronic prescribing. We also use
Docman scanning software to allow efficient scanning and letter management
across both sites.

Proactively
identify and work with a cohort of patients to support their personalised care
requirements, using the available decision support aids. For example those patients with a Learning
Disability, Mental Health, Dementia and vulnerable/frail patients.
- Bring
together all of a persons identified care and support needs and explore their
options to meet these into a single personalised care and support plan (PCSP)
based on what matters to the person, ensuring dignity, choice, respect,
independence and rights are upheld at all times.
- Help
people to manage their needs, answering their queries and supporting them to
make appointments, and ensuring that people have good quality written or verbal
information to help them make choices about their care.
- Identify
barriers to or issues preventing greater engagement and/or uptake by patients
actively participate in developing solutions for patients and service
providers.
- Support
people to take up training and employment, and to access appropriate benefits
where eligible.
- Raise
awareness of shared decision making and decision support tools and assist
people to be more prepared to have a shared decision-making conversation.
- Ensure
that people have good quality information to help them make choices about their
care, e.g. Support people to understand their level of knowledge, skills and
confidence when engaging with their health and wellbeing.
- Assist
people to access self-management education courses, peer support or
interventions that support them in their health and wellbeing.
- Provide
coordination and navigation for people and their carers across health and care
services, alongside working closely with social prescribing link workers,
health and wellbeing coaches and other primary care professionals.
- Support
the coordination and delivery of Multi-Disciplinary Team (MDT) meetings within practice.
- Support
patients with poor attendance records in vulnerable patient groups.
- Support
the paediatric ADHD/Autism (ASD) referral pathway
