an exciting, full-time opportunity for a hardworking, enthusiastic and
innovative Pharmacy Technician to join our growing and established team of Clinical
Pharmacists at Downend Health Group. You will also be working closely with our excellent
Medicines Management Team, CCG Pharmacist and Pre-Registration Pharmacists. No Primary
Care experience? No problem. In addition to the support that you will receive
from our team of Clinical Pharmacists, as part of your training, we will also enrol
you onto an 18-month fully funded CPPE pathway to help you in your development to
ensure that you reach your full potential in primary care. No weekend
commitment, 5 weeks annual leave plus all bank holidays and access to the NHS Pension Scheme.
Main duties of the job
The Pharmacy Technician will support the Clinical Pharmacist and Medicines Management team to continue to provide the following:
- Support the Clinical Pharmacists in the delivery of Clinical Pharmacy services and Chronic Disease management
- Review hospital discharge letters to ensure patient’s medications are appropriately reconciled post-discharge
- Liaise with clinicians and administrative staff to resolve medicine-related queries such as prescription requests for out-of-stock medications
- Encourage safe, cost-effective and evidence-based prescribing in line with the BNSSG formulary
- Ensure all medication monitoring systems and protocols are consistently run and utilised appropriately (e.g. high-risk drugs)
- Monitor practice prescribing and adherence to local and national prescribing guidelines
- Support the completion of the Prescribing Quality Scheme (PQS) and Quality Improvement (QI) audits throughout the year

continues to rise each year. DHG is situated across two large, purpose-built buildings
situated less than one mile apart in the vibrant and up-and-coming Downend area.

The post holder will be
employed by Downend Health Group and work
within their clinical competence as part of a multi-disciplinary team to
provide expertise in medicines management alongside the Clinical Pharmacists.
The role is pivotal to improving the quality of care
and operational efficiencies so will require motivation and passion to deliver
an excellent level of service within the practice.
Medicines Reconciliation
- Reconcile medicines following a hospital admission and work with patients and community pharmacists to
ensure patients receive the appropriate medications post-discharge
- Investigate any unexplained discrepancies on discharge letters by contacting the hospital team directly to rectify any outstanding issues
- Liaise with pharmacy colleagues
in secondary care and be the main point of contact for medicines-related
queries as needed
Medication Reviews
- Improve patient's knowledge of their medications and advise on possible ways to optimise treatments as required
- Reduce inappropriate
poly-pharmacy and wasteful prescribing by undertaking regular targeted medication
reviews
- Pro-actively identify patients
using EMIS searches to identify those who are eligible and due for a
medication review (e.g. polypharmacy, over 75s etc)
- Contribute to a reduction in
medicine-related hospital admissions and readmissions by supporting
patients to get the most from their medicines
- Liaise with colleagues in
community pharmacy and be the main point of contact to provide support and
offer advice as needed
Medicines Safety & Service Development
- Manage patient’s and health
care professionals’ medicine-related queries
- Improve the quality and safety
of prescribing in the practice via the use of mechanisms such as PQS
audits, ECLIPSE and QOF
- Monitor prescribing against
local guidelines to improve the quality, safety and cost effectiveness of
prescribing
- Action drug withdrawals and medication
alerts e.g. MHRA alerts
- Act as a source of medicines
information to all of the practice staff and patients (e.g. medication
doses, side effects, availability, adverse events, possible alternatives
etc)
