Job summary

To maintain an overview of clinical achievement via QOF (Quality and Outcomes Framework) and Enhanced Services and ensure the efficient and timely recall of patients for clinical review.

  • It is essential that candidates:
  • Are experienced in using EMIS Web
  • Have experience in developing and running searches on population reporting
  • Have an excellent understanding of QoF, Enhanced Services and CQRS
  • Have excellent IT skills, literate with Microsoft Word, Microsoft Excel, Outlook and have an ability to adapt to using third-party software
  • Are able to work as part of a team or individually, acting on their own initiative
  • Have good accuracy and attention to detail
  • Have the ability to accurately review data and extract pertinent information
  • Have a clear understanding for the need for strict confidentiality and the protection of personal and sensitive data
  • Have excellent time management skills
  • Have excellent written and verbal communication skills
  • Are committed to professional development
  • Are flexible in their approach

It is desirable that candidates:

  • Have experience with interpreting medical terminology
  • Have educated to A level standard or equivalent.

Downend Health Group was formed in 2018 after the successful merger of two practices; Christchurch Family Medical Centre and Willow Surgery. We are a friendly, rapidly growing PMS practice and both of our surgeries are situated within a short walking distance of each other in Downend, on the outskirts of Bristol. Job responsibilities

A) Core Responsibilities

To be familiar with and maintain up to date knowledge of the Quality and Outcomes Framework (QOF) requirements and associated business rule SNOMED codes

To monitor practice performance against QoF compliance on a monthly, quarterly and annual basis, ensuring the practice maximises income, meets deadlines and fulfils QoF and Enhanced Service requirements

To monitor compliance with local and national enhanced services

To extract data in line with CQRS submission deadlines to support the Practice NHS Contracts Manager in submitting quarterly and annual claims in a timely manner

To design, run searches and prepare clinical information for audit and review

To support and work with the Clinical Teams to identify areas for improvement in data quality

  • Proactively manage and liaise with key contacts to address any areas of potential or actual under performance

To develop, coordinate, implement and manage patient call and recall systems for QoF domains in a timely manner

To support the Clinical Teams in coordinating care for disease prevention targeting patients for assessment and treatment and actively promoting attendance

To oversee the SNOMED coding of all patient clinical documentation and information in relation to QoF

To be responsible for co-ordinating vaccination and immunisation programmes such as baby clinic, childhood immunisations and Influenza vaccination clinics ensuring invites and recalls are sent out in a timely manner.

To promote innovative ideas of improving patient call and recall with particular focus on improving access for our patients.
