Job summary

We are seeking a skilled and compassionate Clinical Pharmacist to join our healthcare team. As a Clinical Pharmacist, you will play a crucial role in providing pharmaceutical care to patients and ensuring the safe and effective use of medications. Your expertise in medication administration, anatomy knowledge, and patient care will contribute to the overall well-being of our patients.

The applicant will be a prescribing clinical pharmacist who acts within their professional boundaries, providing support to general practice staff with regards to managing complex patients.

The post holder will provide support with regards to prescription and medication queries. They will help support the repeat prescription system, deal with acute prescription requests, medicines reconciliation on transfer of care, systems for safer prescribing and provide expertise in clinical medicines advice.

They will improve patients health outcomes and the efficiency of the primary care team by providing direct, accessible and timely medicines and expertise.

The post holder will be an integral part of the general practice team, as well as part of a wider pharmacist network and multi-disciplinary team. They will work to optimise medication issues to improve patient care and safety, and support clinical staff in the management of patients.

As part of their employment, the post holder must hold an independent prescribing qualification.

Main duties of the job

  • Repeat prescription management including dealing with queries raised by our prescriptions co-ordinators.
  • Ensuring patients medication reviews are up to date.
  • Medicines reconciliation from clinical correspondence.
  • Structured medication reviews.
  • Responding to and actioning tasks from clinical and administrative staff.
  • Being responsible for actioning MHRA drug alerts.
  • Involvement with Chronic Disease clinics
  • Signing prescriptions
  • Medication management of patients with long term conditions

The Grange Surgery is looking to recruit an enthusiastic, highly motivated, full-time skilled and compassionate Clinical Pharmacist to join our healthcare team. We are recruiting to expand and support our existing clinical team, this is a permanent position.

We are a well-established, innovative, hard-working and friendly GMS practice based near Birkdale Village in Southport. We are a supportive practice that values staff commitment, team working and communication. We actively encourage staff learning and development that is conducive to practice roles and services we provide.

Our clinical staff are assisted by a friendly and knowledgeable admin/reception team, we receive excellent administration support from the practice team
